p <br /> EXHIBIT A <br /> SCOPE OF WORK <br /> The purpose of this contract is a hazardous materials assessment for seven buildings in the Service <br /> Center Complex located at 3200 Cedar Street. The list Service Center Complex buildings that shall be <br /> assessed are as follows: <br /> Building List <br /> 1. Building No. 1—Public Works Administration <br /> 2. Building No. 2—MVD Main Repair Shop <br /> 3. Building No. 2B—MVD Small Vehicle Repair <br /> 4. Building No. 3—Panama Warehouse <br /> 5. Building No. 4—Public Works Shop <br /> 6. Annex No. 1-Creamery <br /> 7. Annex No. 2—Insurance Building <br /> Scope of Work <br /> The work shall include the required pre-renovation Asbestos Good Faith Inspection (AGFI), including <br /> random lead paint sampling and a light fixture inventory so that hazardous materials:asbestos, lead <br /> mercury and polychlorinated biphenyls, PCB, can be identified for worker protection during any work <br /> activity that may disturb these materials. A breakdown of the scope of work is as follows: <br /> 1. AET RFP 3197 Good Faith Inspection(AGFI) & Hazmat Verification <br /> 2. Lead Paint Chip Survey <br /> 3. Light Fixture Component Estimate <br /> 4. Mold Assessment for Annex No. 2 <br /> Assessment Deliverables <br /> The following items shall be delivered to the City of Everett at the completion of the assessments: <br /> 1. AGFI—1 hard copy per building in separate 3 ring binder <br /> 2. AGFI—Electronic Copy—PDF version <br /> 3. Lead Paint Chip Survey—include results with AGFI per building <br /> 4. Light Fixture Component Estimate—include results with AGFI per building <br /> 5. Mold Assessment for Annex No.2—include results with AGFI per building <br /> 6. Deadline for all deliverables—March 31,2019 <br /> Staff <br /> Alternative Environmental Technologies shall execute the work with the staff as follows: <br /> 1.
