1 <br /> CITY OF EVERETT SPECIAL PROVISIONS <br /> 1-07.3 Forest Protection and Merchantable Timber Requirements <br /> Delete 1-07.3 in its entirety. <br /> 1 1-07.5 Environmental Regulations <br /> 1-07.5(4) Air Quality <br /> 1 Delete all of 1-07.5(4) and substitute the following: <br /> The Contractor shall comply with all rules of the Puget Sound Clean Air Agency <br /> (PSCAA) (800-552-3565). These rules include PSCAA Regulation I. Excerpts of <br /> Regulation I are included in the Appendix D as it relates to fugitive dust control. <br /> The Contractor shall submit a dust control plan including dust control measures <br /> for its activities related to this Contract that may cause dust. This plan shall be <br /> submitted to the Engineer prior to commencing activity at the job site. <br /> 1-07.6 Permits and Licenses <br /> Supplement 1-07.6 by adding the following: <br /> A City of Everett business license is required for the Contractor and the <br /> Contractor's Subcontractors prior to commencing construction on this Contract. <br /> Contractor shall obtain all necessary permits required by law and the City of <br /> Everett. All general building, electrical, plumbing permits will be issued at no cost <br /> to the Contractor. In addition, obtain all required permits for waste disposal sites. <br /> Waste disposal sites shall be in the United States, unless otherwise expressly <br /> stated in the Contract Documents or the City gives prior written approval. <br /> This Project contains less than one acre of total disturbed area within the project <br /> limits and does not require Contractor to apply for Ecology's Construction <br /> IStormwater General Permit. <br /> - 7.9 Wages <br /> 1-07.9(1) General <br /> Delete the first paragraph of 1-07.9(1) and substitute the following: <br /> This Contract is subject to the minimum wage requirements of RCW 39.12 and to <br /> RCW 48.28, as amended or supplemented. Workers shall receive no less than <br /> the prevailing rate of wage. Bidder shall use the Washington State Prevailing <br /> Wage Rates for Snohomish County, effective at the time of bid opening. Bidder is <br /> solely responsible to use the schedule in effect at the Bid Opening Date, <br /> determine the appropriate labor classification(s), and use the appropriate and <br /> correct prevailing wage and benefit rate(s). The hourly minimum rates for wages <br /> and fringe benefits can be obtained at the following URL: <br /> http://www.lni.wa.gov/TradesLicensing/PrevWage/WageRates/default.asp <br /> Printed copies of the current prevailing wage rates are available for viewing at City <br /> of Everett Public Works, 3200 Cedar St, Everett, WA and the City will mail a hard <br /> copy of the prevailing wage rates upon written request received within 7 days of <br /> the Bid Opening Date. <br /> Delete the fifth paragraph of 1-07.9(1) and substitute the following: <br /> If employing labor in a class not listed in the L & I prevailing wage rate schedule, <br /> the correct wage and benefits rate for that class and locality from the Industrial <br /> Statistician, Washington State Department of Labor and Industries (State L&I), <br /> and provide a copy of those determinations to the Project Engineer. <br /> 1-07.9(5) Required Documents <br /> Supplement 1-07.9(5) by adding the following: <br /> Watermain Replacement"T" Division 1 —GENERAL REQUIREMENTS October 10, 2018 <br /> WO No—UP3684 SP—81 <br />
