4IPage <br /> c. Answer questions from the Plan Sponsor, Plan Administrator and Participants concerning requirements, <br /> procedures or benefits of the Plan; <br /> d. Maintain claim reimbursement files; <br /> e. Provide the Plan Administrator with access to the following reports through BPAS' website: <br /> i. Quarterly Plan administration report, including Participant transactions and balances by <br /> investment fund and in total; <br /> ii. Quarterly Trust reconciliation report by Fund and in total; <br /> iii. Plan year-to-date claim activity; <br /> iv. Annual administration report, including Participant transactions and balances by Fund and <br /> in total; <br /> v. Age demographics, including reporting of Participants by configurable age group, <br /> years of service and average account balance; <br /> vi. Claim distribution reports, including Participant census data, claim amounts paid and <br /> withdrawal dates within a selected date range; <br /> vii. Employee census including Participant name, address, dates, and age; <br /> viii. Web usage statistics by a number of categories and the number of web requests <br /> made by Participants within a selected date range, as determined by BPAS and/or <br /> reasonably requested by the Plan Administrator; <br /> f. Perform recordkeeping services as described herein and in accordance with accurate and timely data <br /> provided by the Plan Administrator: <br /> i. Maintain records of allocations of the Plan's assets by Participant and contribution <br /> type for each Fund selected by the Participant or the Plan Administrator, as <br /> applicable; <br /> ii. Perform investment of the Plan Sponsor's contributions and earnings according to the <br /> investment direction and information provided by the Participants or Plan <br /> Administrator; <br /> iii. If applicable, invest existing account balances into Funds according to directions from <br /> Participants provided in accordance with procedures established by BPAS (such <br /> transactions will be processed each business day as requested by Participants and in <br /> accordance with the BPAS' procedures, or, as soon as administratively feasible); <br /> iv. Prepare quarterly master printout of Participant Statements; <br /> v. Respond to telephone inquiries from Participants and the Plan Administrator relating to <br /> day-to-day administration of the Plan; <br /> vi. Prepare annual accounting of Plan assets, to be submitted to the Plan Administrator's <br /> auditors; <br /> vii. Prepare Annual Plan Report; <br /> viii. Provide daily valuation services; <br /> ix. Process contribution amounts; <br /> x. Process and pay claims; <br /> xi. Conduct non-discrimination testing, as applicable; <br /> xii. Maintain records of Participant contributions and income from other sources; <br /> xiii.
