Project: Legion Golf Course Stormwater Detention Project Change Order No. 1 Change Order Effective Date: November 6, 2018 Exhibit A—Description of Changed Work: Change Order No. 1 Modifies 6 bid items to the contract as stated below: Revised Existing Bid Item Bid Item 10-Manhole 60 inch Diameter Type 1. Modify bid item 10 entitled"Manhole 60 inch Diameter type 1", to reduce contract value at unit price of$8,500.00 per each, by removing 4 each, Manhole 60 inch Diameter type I's, resulting in a$34,000.00 decrease to the contract. Bid Item 11-Manhole 72 inch Diameter Type 1. Modify bid item 11 entitled"Manhole 72 inch Diameter type I", to increase contract value at unit price of$11,000.00 per each, by adding 4 each, Manhole 72 inch Diameter type l's, resulting in a$44,000.00 increase to the contract. Bid Item 28-Segmental Retaining Wall. Modify bid item 28 entitled"Segmental Retaining Wall", to increase contract value at unit price of$36.00 per SF, by adding 1,479 SF, resulting in a$53,244.00 increase to the contract. Bid Item 49-Golf Course Tees - Grass. Modify bid item 49 entitled "Golf Course Tees -Grass", to increase contract value at unit price of$5.00 per SF, by adding 2,053.50 SF, resulting in a$10,267.50 increase to the contract. Bid Item 51-Cart Path. Modify bid item 51 entitled "Cart Path", to increase contract value at unit price of$4.50 per SF, by adding 12,626.50 SF, resulting in a$56,819.25 increase to the contract. Bid Item 62-Force Account. Modify bid item 62 entitled "Force Account", to increase contract value by adding $105,000.00, resulting in a$105,000.00 increase to the contract. Exhibit B—Justification: Revised Existing Bid Item Bid Item 10-Manhole 60 inch Diameter Type 1. The 60"manholes were replaced with 72" manholes, Bid Item 11. Bid Item 11-Manhole 72 inch Diameter Type 1. The 72" manholes replaced deleted 60" manholes, Bid Item 10. Bid Item 28-Segmental Retaining Wall. The installed quantity for"Segmental Retaining Wall"over- ran the estimated plan quantity. Bid Item 49-Golf Course Tees - Grass. The installed quantity for"Golf Course Tees - Grass"over- ran the estimated plan quantity. Bid Item 49-Cart Path. The installed quantity for"Cart Path"over-ran the estimated plan quantity. Bid Item 62-Force Account. Additional force account work to dispose of unanticipated additional soil resulted in and over-run of the estimated plan quantity.
