Project: Wetmore Avenue Stormwater Bypass Project <br /> J Yp J <br /> Change Order No.2 <br /> Change Order Effective Date: November 16, 2018 <br /> Total cost of modified bid items: $16,500.00 <br /> JUSTIFICATION <br /> Bid Item #A-2—Traffic Control Labor. This bid item overran plan quantity and is being modified <br /> to account for costs for additional traffic control labor quantities. <br /> New Item #A-103—Cement Concrete Pavement Removal, Including Haul and Disposal. The <br /> intersections of Wetmore Avenue and 35th Street and Rockefeller Avenue and 35th street contained <br /> cement concrete pavement under the existing asphalt, which was unknown at bid time. This new item <br /> is being added to account for costs to pay for the removal, haul, and disposal of cement concrete <br /> pavement at these intersections. <br /> Bid Item#A-104—Roadway Excavation, Including Haul and Disposal. Due to voids under the <br /> street, created from a past water main break,portions of 35th Street broke apart from the construction <br /> equipment activity, which led to more material to be hauled off than was accounted for in the original <br /> scope of work. This new item is being added to account for costs to remove, haul, and dispose of <br /> roadway material on 35th Street. <br /> New Item #A-105—Cement Concrete Curb Ramp. <br /> There were no provisions in the Contract to construct concrete curb ramps, but to maintain ADA <br /> compliance at the intersections of 35th Street and Rockefeller and at 35th Street and Wetmore, it <br /> became necessary to install curb ramps at these two intersections. This new item is being added to <br /> account for costs to install concrete curb ramps. <br /> New Item#A-106— Cement Concrete Pavement. The intersections of Wetmore Avenue and 35th <br /> Street and Rockefeller Avenue and 35th Street contained cement concrete pavement under the asphalt, <br /> which was unknown at bid time. This new item is being added to account for costs to pay for the <br /> replacement of cement concrete pavement at these intersections. <br /> Bid Item#A-107—Additional Cement Concrete Sidewalk. The combined sewer pipe trench <br /> collapsed on the south edge of the trench, due to voids caused by an abandoned wood stave water pipe <br /> and a past broken water pipe, making the trench wider than anticipated. This condition caused more <br /> concrete sidewalk to be removed than anticipated. This new item is being added to account for costs to <br /> pay for additional cement concrete sidewalk for all sidewalk quantities over 125%of plan quantity for <br /> bid item#A-25, as allowed by WSDOT Section 1-04. <br /> Total Cost of Change Order No. 2: $126,002.50 <br /> Contract Time: There are 15 working days being added to this Contract as part of Change <br /> Order No. 2. <br /> Page 4 of 4 <br />
