Page 1 of 5 <br /> EXHIBIT A <br /> Work Order Form <br /> ENVIRONMENTAL SERVICES PROJECT SCOPE AND ESTIMATE <br /> Project Title: Cleanup Support—Former Smith Street Sawmill W.O. #2 <br /> 1. PROJECT SCOPE: <br /> CONSULTANT will complete construction observation environmental services and cleanup support for an <br /> environmental cleanup of the approximate 4.7-acre Former Smith Street Sawmill Property("Property')owned <br /> by the City. The Property is located at 3600 Smith Avenue in Everett, Washington and comprises <br /> Snohomish County Tax Parcel Numbers:00-5760-019-000-05, 00-5760-019-000-06,00-5760-043-000-02, <br /> 00-5760-012-000-03, 29-0529-003-008-00, 29-0529-002-012-00, 29-0529-003-030-00 and <br /> 29-0529-003-010-00. <br /> The City intends to conduct an independent cleanup of the Property under the Model Toxics Control Act <br /> (MICA, Washington Administrative Code (WAC) 173-340), with agency review through Ecology's Voluntary <br /> Cleanup Program (VCP). The City is seeking a "No Further Action" determination from Ecology. <br /> The scope of work includes completing a pre-cleanup data gaps assessment task, preparing plans and <br /> specifications for the cleanup, providing the City with bid support for cleanup contractor procurement, <br /> conducting construction observation and documentation including groundwater compliance monitoring, and <br /> regulatory liaison and closure support for the City in relation to the VCP and NFA request. <br /> Task 1: Data Gaps Assessment <br /> A limited number of soil data gaps that were identified in the RI must be addressed in order to refine the lateral <br /> and vertical extent of the planned remedial excavation cleanup action and to complete cleanup plans and <br /> specifications for contractor procurement. <br /> 1. Update GeoEngineers' health and safety plan for on-site field work. <br /> 2. Contact the public utilities notification service to mark public utilities in the exploration areas, rights-of-way <br /> and easements. In addition, we will subcontract a private utility locating company to mark underground <br /> utilities or obstructions. <br /> 3. Monitor the completion of test pits at the soil contamination delineation data gap locations as identified <br /> in the CAP. Test pits will be completed by a backhoe company under subcontract to GeoEngineers. Soil <br /> generated during test pit excavation will be used to backfill the test pits. <br /> 4. Obtain soil samples from each test pit for field screening and chemical testing. Field screen the soil <br /> samples for evidence of volatiles using visual, water sheen and headspace vapor screening methods. <br /> Visually classify the samples in general accordance with ASTM International (ASTM) D 2488 and maintain <br /> a detailed log of each exploration. <br /> GeoEngineers,Inc. <br /> File No.0661-10803 <br />
