CITY OF EVERETT SPECIAL PROVISIONS <br /> A City of Everett business license is required for the Contractor and the <br /> Contractor's Subcontractors prior to commencing construction on this Contract. <br /> Contractor shall obtain all necessary permits required by law and the City of <br /> Everett. All general building, electrical, plumbing permits will be issued at no cost <br /> to the Contractor. In addition, obtain all required permits for waste disposal sites. <br /> Waste disposal sites shall be in the United States, unless otherwise expressly <br /> stated in the Contract Documents or the City gives prior written approval. <br /> ' 1-07.9 Wages <br /> 1-07.9(1) General <br /> Delete the first paragraph of 1-07.9(1) and substitute the following: <br /> This Contract is subject to the minimum wage requirements of RCW 39.12 and to <br /> RCW 48.28, as amended or supplemented. Workers shall receive no less than <br /> the prevailing rate of wage. Bidder shall use the Washington State Prevailing <br /> Wage Rates for Snohomish County, effective at the time of bid opening. Bidder is <br /> solely responsible to use the schedule in effect at the Bid Opening Date, <br /> determine the appropriate labor classification(s), and use the appropriate and <br /> correct prevailing wage and benefit rate(s). The hourly minimum rates for wages <br /> and fringe benefits can be obtained at the following URL: <br /> http://www.lni.wa.gov/TradesLicensing/PrevWage/WageRates/default.asp <br /> Printed copies of the current prevailing wage rates are available for viewing at City <br /> of Everett Public Works, 3200 Cedar St, Everett, WA and the City will mail a hard <br /> copy of the prevailing wage rates upon written request received within 7 days of <br /> the Bid Opening Date. <br /> Delete the fifth paragraph of 1-07.9(1) and substitute the following: <br /> If employing labor in a class not listed in the L & I prevailing wage rate schedule, <br /> the correct wage and benefits rate for that class and locality from the Industrial <br /> Statistician, Washington State Department of Labor and Industries (State L&I), <br /> and provide a copy of those determinations to the Project Engineer. <br /> 1-07.9(5) Required Documents <br /> Supplement 1-07.9(5) by adding the following: <br /> The Owner may require payroll reports for the Contractor and every Subcontractor <br /> be submitted weekly to the Construction Division, Public Works Service Center, <br /> 3200 Cedar Street, Everett, Washington 98201. The payroll reports shall contain <br /> the.following information: <br /> 1. Name and residence address of each worker. <br /> 2. Social Security number of each worker. <br /> 3. Classification of work performed by each worker. The classification shall <br /> be specific and match the classification categories listed in the Contract <br /> Documents. <br /> 4. Total number of hours employed each day. <br /> 5. Total number of hours employed during the payroll period. <br /> 6. Straight time and overtime hourly rate of wages paid to each worker. <br /> 7. Total or gross amount earned by each worker. <br /> 8.
