Whether fringe benefits were paid to each worker as part of the hourly wage rate or whether fringe benefits were paid into an approved plan, fund, or program. Payrolls may be submitted on Federal payroll form WH-347, or equivalent. The reverse side of the form contains an affidavit that shall be filled out and signed. If the Contractor's payroll reports are computerized, the computerized reports may be submitted along with a Statement of Compliance affidavit photo copied from the back of form WH-347, or equivalent. The first payroll submitted for the Work for both the Contractor and each Subcontractor shall be labeled "Initial." The last payroll submitted for the Work for both the Contractor and each Subcontractor shall be labeled "Final." Payrolls shall be sequentially numbered for all periods in which Work has been done. A certificate of completion for the Work, signed by the City, will constitute 111 acceptance of the Work. The issuance of this certificate of completion will not constitute acceptance of unauthorized or defective Work or material is performed. 1-07.11 Requirements for Nondiscrimination Supplement 1-07.11 as follows: The Contractor will be required to assure that equal employment opportunities will be in effect to all individuals throughout the length of this Contract, pursuant to 1-07.11 REQUIREMENTS FOR NONDISCRIMINATION. Any such complaint shall be in writing and filed with the City's Title VI Coordinator within 180 calendar days following the date of the alleged discriminatory occurrence.
