ARTICLE 10 - INSURANCE BENEFITS <br /> INSURANCE REOPENER: ATU is aware that the City is proposing a consumer driven high <br /> deductible insurance plan, HRA/VEBA coupled with a near site clinic to assist in alleviating the <br /> escalating insurance costs. Accordingly, in order for the parties to address the escalating <br /> insurance costs and proposed plan, the City may reopen Article 10 on or after June 1, 2019. <br /> Section 1: <br /> Effective January 1, 2016, the City agrees to offer medical coverage for eligible employees and <br /> their legal dependents, provided the employee makes a ten percent (10%) contribution toward <br /> the cost of the medical coverage. <br /> If the employee chooses a carrier other than the City's self-insured medical insurance program, <br /> the employee shall pay the premium difference. <br /> Section 2: <br /> The City shall provide $1,000 life insurance per $1,000 of an employee's annual salary rounded <br /> to the next highest thousand for the duration of this Agreement. <br /> Section 3: <br /> The City shall provide the employee accidental death and dismemberment coverage in an <br /> amount equal to that specified in Section 2 hereinabove. <br /> Section 4: <br /> Any employee who wishes to have the group medical coverage and/or the optional insurance <br /> must sign an authorization card to be kept on file with his/her payroll records. If he/she does <br /> not wish to participate, the employee must sign a waiver card for his/her file. <br /> Section 5: <br /> The City shall continue such medical plan payments for a period of up to a maximum of six (6) <br /> months limited to one (1) month's coverage for each full year of service for any employee of the <br /> bargaining unit who is on leave of absence without pay. <br /> Section 6: <br /> The City shall provide to all employees affected by this Agreement and their dependents, who <br /> request it, a dental coverage program through Washington Dental Service that provides 100% <br /> of premium cost of basic dental for the duration of this agreement. Effective July 1, 2007, the <br /> annual maximum shall be increased to $2000.00. <br /> 14 <br />
