Scope of Work Grand Avenue Park Bridge Exhibit A City of Everett Supplement No. 5 5. CIVIL ENGINEERING SUPPORT This supplement extends the services through the construction schedule extension. The assumed additional level of effort is as follows: • Office time support: 8 hours per week for 32 weeks • Site visits: 1 per month for 8 months 6. COMBINED SEWER& STORMWATER SUPPORT No change. 7. ARCHITECTURE&BRIDGE AESTHETICS SUPPORT LMN will provide architectural services as needed. This supplement includes adjustments for the extension of the construction schedule and the following additional tasks: • Guardrail and sandblast mockup • Revised precast stair submittal • Field verification and potential guardrail modifications • Utility Level access coordination • Additional submittal review 8. ELECTRICAL ENGINEERING SUPPORT No change. 9. LANDSCAPE ARCHITECTURE&IRRIGATION SUPPORT No change. 10. ENVIRONMENTAL SERVICES No change. 11. ELEVATOR DESIGN SUPPORT No change. 12. MECHANICAL SUPPORT No change. 13. CONSTRUCTION MANAGEMENT SERVICES The City requested that the project suspend from December 1, 2018 to February 28, 2019, with work resuming March 1, 2019, and projected completion of November 30, 2019. The Contractor Working Days for this project is currently 390 days. Because of the revised construction schedule, the Scope of Work is amended, as follows: I. Upon the execution of Supplement 5,the following changes are in effect: B. Paragraph 13.F.1.a. Is amended as follows: Budget: Staff levels are anticipated in accordance with the attached budget estimate. Consultant services are budgeted for an approximate 35.00 month period, beginning January 2017 through November 30, 2018, with the project suspended December 1, 2018 to approximately February 28, 2019, work resuming approximately March 1, 2019 through November 30, 2019, and WSDOT/FHWA Review between December 1, 2019 and March 30, 2020. iq)ffPage 3 of 4
