*BLACK BOX <br /> Note 3:This quote is offered based upon the MA4000 server(s) being appropriately sized as described in the MA4000 <br /> Release Notes for the latest version of MA4000. <br /> Note 4: Remote access (such as WebEx or VPN)to all Server(s), both old and new, required for duration of project. <br /> Note 5:This quote does NOT include pricing for any advanced features of MA4000 such as MA4000 DAPS (LDAP), <br /> MA4000 ELM or MA4000 Traffic Manager.These items must be quoted under a separate project. <br /> Note 6:This quote is offered based upon an existing and properly functioning MA4000 system, including up to date <br /> successful MA4000 PBX synchronizations, the MA4000 database on the same server as MA4000 and the understanding <br /> that Expense Management is integrated to MA4000 and there is no OW5000 as part of the solution.The effort does not <br /> include any database cleanup or new feature configuration/implementation, it is limited to the relocation of existing <br /> MA4000 database(s) and upgrade of MA4000. <br /> Note 7:This quote is offered based upon the expectation that interruption of MA4000 services can be tolerated, <br /> allowing the upgrade and migration of MA4000 components to take place during standard business hours. <br /> Services Description <br /> NEC Provided Services <br /> - Perform Basic Sanity Testing of MA4000 components involved/impacted by the MA4000 upgrade and <br /> database relocation <br /> -Coordinate migration with integrated applications such as Expense Management <br /> - Install Customer provided SQL Server Standard on new application server(if applicable) <br /> - Necessity of SQL Standard or SQL Express is determined as detailed in the MA4000 installation manual <br /> - LMC lvligration (if applicable) <br /> - Installation/Upgrade and UCE license verification <br /> - NEC CAS Migration (if applicable) <br /> -Application Upgrade on existing server <br /> -Application Installation on new server <br /> - Migrate database file from existing NEC CAS to new NEC CAS <br /> - MA4000 Migration <br /> - h.1igrate database from existing MA4000 <br /> -Application Install/Upgrade and Configuration on new server <br /> - Migrate auxiliary components from existing MA4000_ such as Scheduled Tasks, PBX backups, Custom <br /> Config File Settings <br /> - Basic feature functionality testing for all components installed or upgraded <br /> - Basic Administrative System (MA4000) version differential training (- 2 hours) <br /> Solutions Integrator/Customer Provided Services <br /> - Provide Application Server(s)for MA4000, OW5000 and SQL Server as applicable <br /> - OS with all updates applied <br /> - RAID controller software installed/configured (if applicable) <br /> - Server Hardware Mounting <br /> - Network Connectivity <br /> -Sized appropriately to support all UCE components and configuration listed above <br /> -Onsite technician for duration of project <br /> -Confirm the LMS has licenses sufficient to complete the project and allocated/assigned to this site <br /> - IT network provisioning(TCP1IP connectivity, firewall provisioning, port forwarding. etc.) <br /> - Provide PBX and Voicemail programming for UCE integration(if applicable) <br /> -Make/Provide UCE MA4000 software media available on all Server(s)for installation <br /> - Provide Remote Access (such as WebEx or VPN)to all Server(s)for Installation and training <br /> -Confirm that the existing MA4000 database has been successfully backed up in case a rollback is needed <br /> for any reason <br /> - Provide appropriate SQL Server Standard version media supporting new UCE version install (if applicable) <br /> -Assure MS SQL Server of the old server can be accessed from the new server <br /> - Provide testing assistance for hv1A4000 <br /> - End User Training <br /> Statement of Work Page 12 April 23, 2019 <br />
