• <br /> Section 2 : Licenses Required for Ambulance Service and <br /> Certain Emergency First Aid Services <br /> A. No person shall operate an ambulance service and/or <br /> emergency first aid service unless he obtains an Operator's <br /> License from the City Council of the City of Everett, which <br /> license must be renewed every year. The license shall not <br /> be issued or renewed unless : <br /> 1. The applicant has fulfilled all requirements of <br /> this Ordinance including the consent of inspections as re- <br /> quired and any applicable statutes of the State of Washing- <br /> ton concerning the safety of the ambulance and/or emergency <br /> first aid vehicle , and <br /> 2 . The applicant demonstrates capability and willing- <br /> ness to provide adequate service in the designated area, and <br /> 3 . The applicant demonstrates that each ambulance <br /> and/or emergency first aid vehicle contains the equipment as <br /> required by this Ordinance, and <br /> 4 . The applicant files a copy of a liability insur- <br /> ance policy showing evidence of non-cancellable coverage <br /> in accordance with Section 5 of this Ordinance, and <br /> 5 . The applicant files proof that all Ambulance <br /> Attendants and Conditional Ambulance Attendants in their <br /> employ are certified as hereinafter required by Section 9 , <br /> and <br /> 6 . The Advisory Committee and City Council will <br /> consider the need for ambulance service or emergency first <br /> aid service in the area where the applicant intends to <br /> operate. <br /> Section 3 : Application for Ambulance Operator' s <br /> License - Emergency First Aid Service License <br /> Applications for ambulance operators and/or emergency <br /> first aid service licenses hereunder shall be made at <br /> the office of the City Clerk upon such forms as may be <br /> prepared or prescribed by the License Officer and shall <br /> contain: <br /> A. The name and address of the applicant and of the own- <br /> er of the ambulance and/or emergency first aid vehicle. <br /> B. The trade or other fictitious name , if any, under <br /> which the applicant does business and proposes to do <br /> business . <br /> C. A description of each ambulance or emergency first <br /> aid vehicle , including the name, model, year of manufac- <br /> ture, the length of time the vehicle has been in use, <br /> and the color scheme, insignia, name, monogram or other <br /> distinguishing characteristics to be used to designate <br /> applicant' s ambulance or emergency first aid vehicle. <br /> D. The location and descriptions of the place or places <br /> from which it is intended to operate. <br /> E. Such other information as the License Officer shall <br /> deem reasonably necessary to a fair determination of <br /> compliance with this ordinance . <br /> F. An accompanying license fee of $100 . 00 , except in <br /> the case of City of Everett departments operating emer- <br /> gency first aid service. <br />
