B. The License Officer shall have authority to summarily revoke any license or conditional license on any of the following grounds , 1. If the license holder is convicted of, or for- feits bail for, any of the following driving violations whether or not in connection with driving an ambulance: a. Reckless driving. b. Driving under the influence of intoxicat- ing liquor or drugs . 2 . If the license holder is convicted of any felony or any offense involving moral turpitude. 3 . If the license was obtained by fraud or if the application therefor contains any false statements, or if a fact concerning the applicant was not disclosed at the time of his making application when such fact would have constituted just cause for refusing to issue such license. 4 . The decision of the License Officer is subject to review by the City Council when written notice is filed with the License Officer within ten (10) days of notice of the decision of the License Officer. C. The initial , semi-annual, or other ambulance and/or emergency first aid vehicle , equipment and premise inspec- tion reports of the License Officer herein provided for shall be prima facie evidence of compliance or non-compli- ance with, or violation of, the provisions , standards and requirements provided herein, and ambulances and/or emer- gency first aid vehicles . D. Upon suspension, revocation or termination of an am- bulance and/or emergency first aid vehicle license here- under operations as such shall cease with such vehicle and no person shall permit continued operation with such ambulance and/or emergency first aid vehicle as such. Upon suspension revocation or termination of an Ambulance Attendant Emergency Medical Technician' s or Conditional Ambulance Attendant Emergency Medical Technician' s license hereunder, such Ambulance Attendant Emergency Medical Technician or Conditional Ambulance Attendant Emergency Medical Technician shall cease to drive or attend an am- bulance and/or emergency first aid vehicle and no person shall employ or permit such individual to drive or attend an ambulance and/or emergency first aid vehicle until deemed requalified as pursuant to this ordinance and upon review of the License Officer. Section 12 : Reports A. Each licensee of an ambulance and/or emergency first aid vehicle hereunder shall maintain accurate records , upon such forms as may be provided or prescribed by, and containing such information as may be required by, the License Officer, concerning the transportation of each patient within the City of Everett or from one place herein to another place within or beyond its limits . Such records shall be available for inspection by the License Officer at any reasonable time .
