B. The License Officer shall have authority to summarily <br /> revoke any license or conditional license on any of the <br /> following grounds , <br /> 1. If the license holder is convicted of, or for- <br /> feits bail for, any of the following driving violations <br /> whether or not in connection with driving an ambulance: <br /> a. Reckless driving. <br /> b. Driving under the influence of intoxicat- <br /> ing liquor or drugs . <br /> 2 . If the license holder is convicted of any felony <br /> or any offense involving moral turpitude. <br /> 3 . If the license was obtained by fraud or if the <br /> application therefor contains any false statements, or if <br /> a fact concerning the applicant was not disclosed at the <br /> time of his making application when such fact would have <br /> constituted just cause for refusing to issue such license. <br /> 4 . The decision of the License Officer is subject <br /> to review by the City Council when written notice is filed <br /> with the License Officer within ten (10) days of notice <br /> of the decision of the License Officer. <br /> C. The initial , semi-annual, or other ambulance and/or <br /> emergency first aid vehicle , equipment and premise inspec- <br /> tion reports of the License Officer herein provided for <br /> shall be prima facie evidence of compliance or non-compli- <br /> ance with, or violation of, the provisions , standards and <br /> requirements provided herein, and ambulances and/or emer- <br /> gency first aid vehicles . <br /> D. Upon suspension, revocation or termination of an am- <br /> bulance and/or emergency first aid vehicle license here- <br /> under operations as such shall cease with such vehicle <br /> and no person shall permit continued operation with such <br /> ambulance and/or emergency first aid vehicle as such. <br /> Upon suspension revocation or termination of an Ambulance <br /> Attendant Emergency Medical Technician' s or Conditional <br /> Ambulance Attendant Emergency Medical Technician' s license <br /> hereunder, such Ambulance Attendant Emergency Medical <br /> Technician or Conditional Ambulance Attendant Emergency <br /> Medical Technician shall cease to drive or attend an am- <br /> bulance and/or emergency first aid vehicle and no person <br /> shall employ or permit such individual to drive or attend <br /> an ambulance and/or emergency first aid vehicle until <br /> deemed requalified as pursuant to this ordinance and upon <br /> review of the License Officer. <br /> Section 12 : Reports <br /> A. Each licensee of an ambulance and/or emergency first <br /> aid vehicle hereunder shall maintain accurate records , <br /> upon such forms as may be provided or prescribed by, and <br /> containing such information as may be required by, the <br /> License Officer, concerning the transportation of each <br /> patient within the City of Everett or from one place <br /> herein to another place within or beyond its limits . <br /> Such records shall be available for inspection by the <br /> License Officer at any reasonable time . <br />
