EXHIBIT 1 <br /> Example - EnrollmenFile Data Specifications and Requirements <br /> Objective <br /> The purpose of this specifications document is to outline the methodology and requirements for <br /> the Enrollment File document. The file is required and identifies all eligible members who will <br /> be using Vera Whole Health services. Information included on the Enrollment File is uploaded <br /> into our Electronic Health Record(EHR) system by the Enrollment Utility Software. This <br /> software automates the proess of uploading a large amount of data in bulk into our EHR. <br /> Key Steps <br /> 1. Timely delivery of he Enrollment File is critical. During the onboarding phase,the `Final <br /> Test Enrollment' file needs to be received six weeks prior to launch date. Subsequently, <br /> the Enrollment File need to be received by the 3rd business day of the month. This will <br /> ensure that all mem ers eligible for Vera services will be uploaded into the EHR for any <br /> given month. <br /> 2. The Enrollment Fil needs to be submitted in a standardized format for the Enrollment <br /> Utility to upload th file. The utility will reject a file that is not in the specified format. <br /> Follow the tips shown below to prepare a bulk enrollment file for upload: <br /> Data Transmission <br /> ❖ Vera Whole Health prefers to receive the Enrollment Files by Vera downloading them <br /> from your Secure FTP site;In this case, Login Credentials are needed for Vera to access <br /> your site. Alternatively, Enrollment Files can be uploaded to Vera's site, Egnyte; login <br /> information will be provided. <br /> Data Format <br /> FILE-LEVEL SPECIFICATIONS <br /> ❖ All enrollment files must be in Comma Separated Value (.csv) format. <br /> Exhibit 1, Page 1 <br />
