A touchpoint can include for example: a <br /> provider visit,a bioscreen or flu shot,a group or individual coaching encounter,and can <br /> occur in or out of a Vera clinic. <br /> The Employee engagement percentage is calculated as the number of employee Participants <br /> who have engaged with Vera during the Clinic Year divided by the average number of eligible <br /> employee Participants for that same Clinic Year,with the result multiplied by 100. <br /> (2) The Annual Whole Health Evaluation is defined as the completion of a 4 part visit(or series <br /> of visits)made up of: 1)a provider wellness visit,2)introduction to coaching visit,3) <br /> biometric screen,and 4)completion of Vera's health risk assessment questionnaire. <br /> Employer may choose to eliminate the completion of the health risk assessment and make <br /> the Annual Whole Health Evaluation only the other three parts. <br /> The Annual Whole Health Evaluation completion percentage is calculated as the number of <br /> Participants who have completed the Annual Whole Health Evaluation in the Clinic Year <br /> divided by the average number of Participants for that same Clinic Year, with the result <br /> multiplied by 100. <br /> Schedule 2, Exhibit C-3 <br />
