Pre-Demolition Regulated Building Materials Assessment and Cost Estimate irerracon <br /> 2600 Federal Avenue • Everett, Washington <br /> July 3, 2019 •Terracon Proposal No. P81197291 <br /> Terracon understands that the intent of the assessment is to assist the Client with <br /> communicating the presence of regulated building materials and the presence, location, and <br /> quantity of ACM to employees, vendors, and contractors working in the building and to meet the <br /> requirements for an asbestos survey for the Puget Sound Clean Air Agency (PSCAA) and a <br /> good faith inspection as required by Washington State Department of Labor and Industries' <br /> Division of Occupational Safety and Health (DOSH) regulations prior to demolition. <br /> The proposed Scope of Services and fee presented below are based on telephone <br /> conversations and a site walk with Ms. Christina Anderson of Terracon and Mr. Mark Sadler of <br /> the City of Everett on May 29, 2019. <br /> SCOPE OF SERVICES —ASBESTOS ASSESSMENT <br /> In conducting the asbestos assessment, Terracon will: <br /> • Provide a Project Manager who is an Asbestos Hazard Emergency Response Act <br /> (AHERA)-accredited building inspector and project designer to develop an appropriate <br /> sampling plan to meet the needs of the project, review the report, and manage the project. <br /> • Provide an Industrial Hygienist, who is an AHERA-accredited building inspector to <br /> Conduct a good faith inspection for asbestos in accordance with Washington <br /> Administrative Code (WAC) 296-62 and an asbestos survey in accordance with PSCAA <br /> Regulation III, Article 4 including a walk-through of the project area to identify accessible <br /> building materials suspected of containing asbestos. <br /> u Sample accessible materials likely to contain asbestos in general accordance with the <br /> AHERA-based sampling protocol (40 Code of Federal Regulations [CFR] 763.86). It is our <br /> understanding that the building will not be occupied at the time of the assessment, and the <br /> Client has authorized limited destructive sampling methods, however, samples will be <br /> collected from discreet locations. Sample locations will be cleaned but not repaired. <br /> Suspect ACM that are assessed but not sampled will be listed as assumed ACM, if <br /> present or suspected to be present, in our report. <br /> • Terracon will collect roof samples (if necessary) and will provide temporary roof patching <br /> and/or will coordinate with the Client's maintenance personnel for roof patching. Our <br /> personnel are not certified in roofing repair, and we shall therefore under no circumstances <br /> be responsible for voiding the roof warranty, or the adequacy and water tightness of the <br /> temporary repairs, nor shall we be responsible for any water damage to the roofing <br /> system, building, or its contents resulting from our temporary repairs. <br /> Responsive • Resourceful • Reliable 2 <br />
