On October 31, the Metro Center team had a phone meeting with Hil Kaman, George Kosovich <br /> (Assistant Superintendent of Verdant Health)and Mallory Taylor(Community Outreach and Enrollment <br /> Manager for Community Health Center of Snohomish County)to further discuss the request and the <br /> needs of the program. <br /> Proposed Process <br /> Developing an evaluation will first require building consensus among CHART partners to define project <br /> outcomes. Doing so establishes the framework for the evaluation and helps define the pathway that <br /> CHART participants take as they progress through the program.The process of building consensus <br /> begins in the Pre-Assessment and Assessment phases described below. Once outcomes are identified, <br /> we can determine how and when they can be measured and assess the economic impacts of the <br /> program during the Implementation phase. <br /> Pre-Assessment <br /> A Pre-Assessment helps us accurately gauge the scope and question design of the Assessment phase <br /> including the number of individuals to be engaged, common themes and differences. <br /> The Metro Center conducted a Pre-Assessment in January/February 2019. Kevin Harris completed <br /> interviews with representatives from five core organizations to discuss the general goals and perception <br /> of the proposed project, and to identify which key stakeholders should be engaged in the Assessment <br /> phase. <br /> Assessment <br /> The Assessment will identify potential consensus points among stakeholders around CHART program <br /> outcomes and how they will be measured, as well as identify potential processes that will yield the best <br /> information to create the evaluation design. Using feedback obtained from pre-assessment <br /> interviewees, we will determine a roster of 20-25 stakeholders for informal one-on-one assessment <br /> interviews.We will send questions to participants in advance for their thoughtful consideration. <br /> Participants will be informed that their names will not be associated with their opinions and statements, <br /> to enable frank and open conversation. <br /> The common themes (and differences)that emerge from these interviews will help us design one or two <br /> group convenings for up to 75 participants and respective processes,to address the following: <br /> 1. Define the CHART program, including specific objectives, participant requirements, roles of each <br /> participating entity and the timeline for delivery of the CHART program. <br /> 2. Define the expected outcomes, how and when each outcome will be measured, and how many <br /> times each outcome will be measured during delivery of the program. <br /> 3. Consider potential good and bad unintended consequences, such as lower emergency <br /> department use, but greater primary care use, and how to capture the costs or benefits of such <br /> consequences. <br /> 4. Establish a pathway to convert outcomes into success or failure, and whether a threshold <br /> number of failures within the delivery timeline will result in disqualification from the program. <br /> 5. Establish a method to use success/failure rates, cost of delivery of the program,and <br /> costs/benefits of intended and unintended consequences into the program evaluation. <br /> Additionally, we will assess the availability and utility of data for use in the evaluation. <br /> 3/15/2019 MetroCenter.wsu.edu Page 2 of 3 <br />
