EVERETT WASHINGTON City of Everett Human Needs Grant January 1, 2019 through December 31, 2019 Exhibit B Terms, Conditions and Projected Budget Organization&Program: Housing Hope: Commerce Bldg. Housing&Social Services for People with Disabilities Will spend the funds received for the specific purposes of: Providing housing and social services for the low-and very low-income single individuals residing at the Commerce Building in downtown Everett. In addition to their low incomes,these residents also struggle with chronic mental and physical disabilities that significantly limit their options for increased income and negatively impact their ability to maintain stable housing. For the purposes of this grant,a unit of service is defined as: A Resource Coordination unit of service is defined as a face-to-face substantive interaction or contact with a client for 10 minutes or longer. Housing Hope: Commerce Bldg. Housing&Social Services for People with Disabilities anticipates this grant will fund a total of 680 units of service for 55 (unduplicated count) Everett residents each year(define period—month, quarter, etc.) Housing Hope: Commerce Bldg.
