Wholesale Distributor's Amusement $500.00 per annum <br /> Device License <br /> Amusement Arcade License 200.00 per annum <br /> Location Amusement Device License 10.00 for each device <br /> BDevicesLicenseLocation Amusement 15.00 for each device <br /> Operator s Amusement Device Sub- <br /> License 10.00 for each device <br /> per annum <br /> Operator's Amusement Device Master <br /> License 500.00 per annum <br /> However, the fee for "Location Amusement Device License" and "Operator's Amusement <br /> Device Sub-License" shall be pro-rated at one-half the yearly fee as of June 1. <br /> Applications for the Wholesale Distributor's Amusement Device License, Amusement <br /> Arcade License, Operator's Amusement Device Master License, or transfer thereof, <br /> shall be referred to the City Council which may grant or refuse the same, and <br /> its decision thereon shall be final and conclusive. If the City Council approves <br /> the granting or transfer of the license, it shall by resolution direct the City <br /> Clerk-Comptroller to issue or transfer the license, which shall be held subject to <br /> all provisions of this ordinance. An operator of amusement devices shall not have <br /> in use, play or operation in the City of Everett more than two hundred (200) such <br /> devices at any one time; not more than eight (8) sub-licenses shall be issued for <br /> any location except in connection with an "Amusement Arcade License" under section <br /> 2 of this ordinance. Any operator of amusement devices who shall apply for and <br /> receive any Amusement Device Sub-License plate for use at specified location, <br /> shall, when the use of such plate is discontinued, return same within five (5) days <br /> to the City Clerk-Comptroller. <br /> Application for all licenses provided for herein shall be made to the City Clerk- <br /> Comptroller accompanied by a receipt of the City Treasurer showing that the applicant <br /> has paid the required fee for the license, and shall be made on forms to be prescribed <br /> by the City Clerk-Comptroller. <br /> If the City Clerk-Comptroller finds that the license applied for is for sale, lease <br /> or display of a device or machine which may be legally licensed, sold, displayed and <br /> operated under the provisions of, and according to the standards laid down by this <br /> ordinance and such application has been approved by the City Council, he shall issue <br /> to the applicant the license applied for, and shall notify the City Treasurer, the <br /> latter shall thereupon pay into the General Fund of the City Treasury the fey received <br /> from the applicant; otherwise, the application shall be rejected by the City Clerk <br /> and the fee refunded. <br /> Section 7 . NAME OF WHOLESALE DISTRIBUTOR AND OPERATOR TO BE ON AMUSEMENT DEVICE: <br /> It shall be unlawful for any person to sell, exchange, display or exhibit for use or <br /> operation, or to lease, rent to, or place with another for display, exhibition, use or <br /> operation, any amusement device unless the same shall have indelibly printed, painted, <br /> stamped or impressed thereon the name and address of the Wholesale Distributor; <br /> and, if rented, leased, displayed or exhibited by an operator, the name and address <br /> of the operator shall be indelibly printed, painted, stamped or impressed thereon. <br /> Section 8 . PLAY BY MINORS PROHIBITED: It shall be unlawful for anyone under the <br /> age of twenty-one (21) years to play or operate any amusement device as defined by <br /> this ordinance; and it shall be unlawful for any operator or any owner of any such <br /> amusement device or for any manager or other person in charge of premises where any <br /> such amusement device is kept, maintained or operated, to permit or allow any person <br /> under twenty-one (21) years of age to play or operate any amusement device; provided <br /> that such prohibition shall not apply where no extra games, plays, rewards, or things <br /> of value are given by or in connection with the operation of any coin-operated <br /> amusement device. <br />
