6 <br /> 2.5 CLEAN-UP AND DISPOSAL <br /> The City expects the following: <br /> • Debris from tree trimming,tree removal, and stumping operations shall be cleaned up <br /> each day before the work crew leaves the site unless otherwise authorized by the City. <br /> • Lawn areas shall be raked,streets and sidewalks shall be swept,and brush, branches <br /> and logs shall be removed from the site. <br /> • In open areas the tree clean-up may be established to chip in place limbs,cut tree into <br /> lengths chip piles and stacked wood temporarily left on site as approved by the city. <br /> • Materials left on site temporarily must be secured in a'manner such that it does not <br /> present a hazard the public. <br /> • Final clean-up areas are to be left in a condition equal to that which existed prior to the <br /> commencement of forestry operations. <br /> • Disposal must be at a legal site with the proper permitfor'handling such waste <br /> 2.6 PREVAILING WAGE REQUIREMENTS \ \� <br /> This Request for Proposal includes work that is subject to prevailing wages as required by RCW <br /> 39.12 and RCW 49.28 (Hours of Labor)as'amended or supplemented. <br /> The wage rates to be paid to all laborers,workers;'and mechanics that perform any part of this <br /> Contract shall not be less than the prevailing wage rates as required by Chapter 39.12 of the <br /> Revised Code of Washington,as amended:.This rquirementapplies to laborers,workers,and <br /> mechanics whether they are employed by the Contractor,Sub Contractor,Sub-subcontractors, <br /> or any other person who performs a portion of the Work completed by this Contract. <br /> The State of Washington prevailing wage rates applicable for this solicitation,which is located <br /> in Snohomish County, may be found at the following website address for the Department of <br /> Labor& Industries:https://fortress.wa.gov/Ini/wagelookup/ <br /> In ref re\ing such rates,The City of Everett does not imply or warrant that the Supplier will <br /> find labor available at those Tates. It is the Supplier's sole responsibility to determine the wage <br /> rates actually'paid. • <br /> 2.7 INTENTS&AFFIDAVITS <br /> Upon receipt of the required documents,the awarded contractor will receive a purchase order <br /> which will serve as the contract. Purchase order number and department or requester full name <br /> must be on all invoices. <br /> At the start of each project,the contractor(and any subcontractors if applicable) must file a <br /> Statement of Intent to Pay Prevailing Wages with the Industrial Statistician of the Department of <br /> Labor and Industrial Services(DLIS). The Department of Labor and Industries charges a fee for <br /> such approval and certification,which shall be paid by the Contractor. Any change in the fee will <br /> not be grounds for revision of the Contract Sum. <br /> 40 9 <br />
