No payment will be made on this contract until the contractor has submitted a 'Statement of <br /> Intent to Pay Prevailing Wages'that has been approved by the Department of Labor and <br /> Industries. At the end of each contract year,the supplier shall submit an Affidavit of Wages <br /> Paid'that has been approved by the Department of Labor and Industries. The City of Everett <br /> may hold payments until the contractor(and each and every subcontractor if applicable) has <br /> submitted an 'Affidavit of Wages Paid' and a new Statement of Intent to Pay Prevailing Wages <br /> that has been approved by the Department of Labor and Industries for each project. With each <br /> invoice,the Contractor will attach or write a statement that wages <br /> paid were in compliance with applicable Prevailing Wage rates. <br /> 2.8 SUPPLIER RESPONSIBILITIES <br /> The Contractor must provide: <br /> • Obtain all necessary licenses and perrtiits needed to conduct the work required under <br /> the terms of this contract at their expense. <br /> • Obtain all necessary lane closure,.property owner access and other needed <br /> authorization to conduct tree maintenance operations on rights-of-way,easements or <br /> city owned property. <br /> • Provide any and all necessary formal notices required in conjunction with the execution <br /> of work. <br /> • Provide all supervision, labor,tools,equipment,materials,supplies,traffic control, <br /> safety measures,and services <br /> • Provide trucks,devices,chippers,stump grinders, hand tools, and any other equipment, <br /> machinery and sup\pfies necessary to perform the work requested <br /> • Removing-and disposing of all wood brush and debris resulting from the tree <br /> maintenance operations unless the city indicates that the property owners request the <br /> wood be leftonsite <br /> 2.9 TRAFFIC CONTROL <br /> The city may provide traffic control as mutually agreed upon. Closure of public streets will not <br /> be permitted unless prior arrangements have been approved by the City of Everett Traffic <br /> Engineer. Traffic control must be accomplished in accordance with the Manual on Uniform <br /> Traffic Control Devices(MUTCD), https://mutcd.fhwa.dot.gov/. <br /> 2.10 WORKING HOURS& RESPONSE TIME <br /> Work performed by the Contractor must be in compliance with the City of Everett Noise <br /> Ordinance, EMC 20.08 and related sections and scheduled between the hours of 7:00 AM to <br /> 6:00 PM, Monday through Friday, or 8:00 AM to 6:00 PM weekends. The City may authorize <br /> work outside these hours when necessary for emergency tree maintenance. <br /> A. Normal Response Time for Tree and Stump Removal-The City of Everett will <br /> coordinate with the Supplier to establish a reasonable response time for the given <br /> 41 10 <br />
