No payment will be made on this contract until the contractor has submitted a 'Statement of Intent to Pay Prevailing Wages'that has been approved by the Department of Labor and Industries. At the end of each contract year,the supplier shall submit an Affidavit of Wages Paid'that has been approved by the Department of Labor and Industries. The City of Everett may hold payments until the contractor(and each and every subcontractor if applicable) has submitted an 'Affidavit of Wages Paid' and a new Statement of Intent to Pay Prevailing Wages that has been approved by the Department of Labor and Industries for each project. With each invoice,the Contractor will attach or write a statement that wages paid were in compliance with applicable Prevailing Wage rates. 2.8 SUPPLIER RESPONSIBILITIES The Contractor must provide: • Obtain all necessary licenses and perrtiits needed to conduct the work required under the terms of this contract at their expense. • Obtain all necessary lane closure,.property owner access and other needed authorization to conduct tree maintenance operations on rights-of-way,easements or city owned property. • Provide any and all necessary formal notices required in conjunction with the execution of work. • Provide all supervision, labor,tools,equipment,materials,supplies,traffic control, safety measures,and services • Provide trucks,devices,chippers,stump grinders, hand tools, and any other equipment, machinery and sup\pfies necessary to perform the work requested • Removing-and disposing of all wood brush and debris resulting from the tree maintenance operations unless the city indicates that the property owners request the wood be leftonsite 2.9 TRAFFIC CONTROL The city may provide traffic control as mutually agreed upon. Closure of public streets will not be permitted unless prior arrangements have been approved by the City of Everett Traffic Engineer. Traffic control must be accomplished in accordance with the Manual on Uniform Traffic Control Devices(MUTCD), https://mutcd.fhwa.dot.gov/. 2.10 WORKING HOURS& RESPONSE TIME Work performed by the Contractor must be in compliance with the City of Everett Noise Ordinance, EMC 20.08 and related sections and scheduled between the hours of 7:00 AM to 6:00 PM, Monday through Friday, or 8:00 AM to 6:00 PM weekends. The City may authorize work outside these hours when necessary for emergency tree maintenance. A.
