7. Maintain all licenses and qualifications required to perform psychological <br /> evaluations in accordance with the NFPA and notify the city immediately if the <br /> status of their license or qualifications changes in a manner that no longer meets <br /> these standards. <br /> 2.5 CITY OF EVERETT RESPONSIBILITIES <br /> • Identify candidates and notify the Supplier in writing <br /> • Coordinate an appointment for the candidate <br /> • Notify the candidate of the appointment <br /> 2.6 FITNESS FOR DUTY <br /> Fee schedule for a complete fitness for duty report. The evaluation shall include <br /> appropriate testing and interview. The report will include an opinion of the employee's <br /> ability to handle routine duties and written recommendations. <br /> 2.7 DESIRED ADDITIONAL SERVICES <br /> On an as needed basis, provide expert legal testimony under subpoena or formal City <br /> Government notification for legal hearings which might include, City Courts, Municipal or <br /> Magistrate Court, State Courts, Superior Courts, Federal Courts, Administrative Judicial <br /> hearings, Public Review Board Hearings, and or a formal request to appear before the <br /> City of Everett Council to include Council working sessions. <br /> On an as needed basis, provide training to staff on topics which may include coping <br /> mechanisms, psychological resilience and how to read the psychological reports. <br /> 2.8 SUPERVISION <br /> The Supplier will report to the Fire Chief. Check-ins and consultation as determined by <br /> that person. <br /> 2.9 AUDITS AND INSPECTIONS <br /> At any time during normal business hours and as often as the City may deem necessary, <br /> Supplier shall make available to the City for the City's examination all of Supplier's books, <br /> records and documents with respect to all matters covered by this Agreement and, <br /> furthermore, Supplier will permit the City to audit, examine and make copies, excerpts or <br /> transcripts from such records, and to make audits of all contracts, invoices, materials, <br /> payrolls, records of personnel, conditions of employment and other data relating to all <br /> matters covered by this Agreement. <br /> 2.10 PRICE INCREASES <br /> Increases to the contract pricing may be requested by the Supplier not more than once <br /> per year, and shall be accompanied by sufficient documentation to allow the City of <br /> Everett to analyze the validity of the request for increase. Such documentation shall be <br /> presented to the City's Purchasing Manager not less than 60 days before the anniversary <br /> of the contract, or the date of commencement of optional years of the contract term. Any <br /> such contract increase shall only be effective upon written acceptance issued by the City <br /> of Everett to the Supplier. <br /> 8 <br />
