Project: Everett Downtown Streetscape Improvement <br /> Change Order No.1 <br /> Change Order Effective Date: October 16, 2019 <br /> Bid Item# E-5—Gravel Borrow for Trench Backfill. Modify existing bid item#E-5 entitled, <br /> "Gravel Borrow for Trench Backfill"at the Contract unit price of$44.00/Ton,by adding 425 tons to <br /> the Contract, resulting in a Contract increase of$18,700.00. <br /> Bid Item# E-6—Ductile Iron Pipe for Water Main, 8-Inch Diameter. Modify existing bid item# <br /> E-6 entitled, "Ductile Iron Pipe for Water Main, 8-Inch Diameter" at the Contract unit price of <br /> $110.00/Linear Foot,by adding 125 linear feet to the Contract, resulting in a Contract increase of <br /> $13,750.00. <br /> Bid Item# E-16—Water Service, 2-Inch. Modify existing bid item#E-16 entitled, "Water Service, <br /> 2-Inch" at the Contract unit price of$55.00/Linear Foot,by adding 100 linear feet to the Contract, <br /> resulting in a Contract increase of$5,500.00. <br /> JUSTIFICATION <br /> Bid Item#A-17—Roadway Excavation, Including Haul. This bid item overran plan quantity and is <br /> being modified to account for additional quantities of roadway excavation, including haul. <br /> New Item#A-101 —Equitable Adjustment for Frontier Vault Lid Delay on Hoyt Avenue. An <br /> unknown Frontier Communications Vault on Hoyt Avenue was higher than the new concrete road <br /> elevation, so a new concrete vault lid was ordered to match the new elevation,but the new lid took six <br /> (6) weeks to arrive. To keep work going, the contractor worked around the vault, changing formwork, <br /> adding more concrete placements to the schedule, and reconfiguring the pavement steel and jointing <br /> plan. This new item is being added to account for costs of additional labor and equipment due to the <br /> inefficiencies created by these unforeseen conditions. <br /> New Item#A-102—Delete Electrical work on Hoyt Avenue. City of Everett Traffic Engineering <br /> determined that the underground electrical conduit to be installed across Wall Street at Hoyt Avenue <br /> was unnecessary, as were seven(7)junction boxes to be installed on Hoyt Avenue, and are being <br /> deleted from the Contract. This item is added so the City of Everett can receive an appropriate credit to <br /> the Contract for deleting this work <br /> New Item#A-103—Equitable Adjustment for Pre-Formed Loop Delay on Hoyt Avenue. The <br /> Contract required that the vehicle detector loops on Hoyt Avenue be sawcut in the pavement,but since <br /> the City of Everett decided to install Cement Concrete Pavement instead of asphalt, the COE Traffic <br /> Engineering Division required that pre-formed loops be installed within the cement concrete pavement. <br /> This change required that the Contractor revise, reschedule, and add to their concrete pavement work <br /> while waiting on loop approval from Traffic Engineering, resulting in additional labor and equipment <br /> costs. This new item is being added to account for costs for this change to the Contract. <br /> New Item# B2-10—Hoyt Avenue Mid-Block Crosswalk Change. The Contract plans and <br /> specifications did not require any coloring agent to be added to the mid-block crosswalk on Hoyt <br /> Avenue, even though the previous concrete crosswalks installed on Hoyt Avenue in downtown Everett <br /> did. This item is being added to pay for costs to add Davis 860 Gray coloring agent to the concrete <br /> Page 5of7 <br />
