Rehabilitation of housing units owned/operated by a public housing authority <br /> (PHA). <br /> Description of how the project Activity will meet this subcategory: EHA wholly owns and <br /> operates Evergreen Cottages as very low-income housing for the elderly. Based on <br /> EHA's asset management tools, capital needs assessments and field evaluations, it was <br /> determined that the property's building envelope and roofing systems were at the end of <br /> its useful life. The existing siding was failing in numerous locations, causing water <br /> intrusion and interior envelope damage. Roofs, gutters and flashing on all five buildings <br /> are also in need of replacement. EHA is anticipating replacing building's envelope and <br /> roof with similarly or better rated materials. These improvements will help ensure the <br /> property's physical viability for over the next 20 — 30 years. <br /> C. Performance Monitoring <br /> The City will monitor the performance of the Subrecipient against goals and performance <br /> standards required herein through quarterly reports and site visits. Substandard <br /> performance as determined by the City will constitute non-compliance with this Agreement. <br /> If action to correct such substandard performance is not taken by the Subrecipient within a <br /> reasonable period of time, as determined by the City and after being notified by the City, <br /> contract suspension or termination procedures will be initiated. <br /> D. Levels of Accomplishment - Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the <br /> Subrecipient agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Housing preservation by the <br /> replacement of the 20 20 <br /> property's building envelope <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 20 senior households whose gross annual <br /> household income is not more than fifty 20 households <br /> percent (50%) of the annual median income <br /> (at the time of initial occupancy) <br /> *Note: Provide definition of Units of Service, which may include such measures as units <br /> rehabbed, persons or households assisted, or meals served, here.
