4. Set up a phone line or lines for potential and current clients to call; Respond <br /> to current clients within two hours or whatever timeframe is appropriate; <br /> 5. Accept appropriate clients who are in need of parking; <br /> 6. Request that clients fill out an Informed Consent for Release of Information (ROI) <br /> to appropriate staff at Interfaith Family Shelter and CVPC for the purpose of <br /> coordination of care; CVPC shall be consulted in the drafting of such an ROI <br /> and shall have editing authority; <br /> 7. Provide clients' signed Code of Conduct forms, confidentiality policies, and <br /> other documents to CVPC and agree to work with clients in accordance with these <br /> principles and practices; <br /> 8. Encourage and receive program referrals and screen potential clients; <br /> screening will include elimination of identified sex offenders and people <br /> identified as persons who were convicted of violent offenses through the <br /> Washington State Patrol's Watch Report; <br /> 9. Conduct intake which will include review, background checks and signing <br /> Code of Conduct, providing parking permit for vehicle, assigning parking <br /> space, and assigning key code for building access; <br /> 10. Provide supportive services and housing and employment- <br /> focused case management; meet with clients regularly, at least <br /> weekly; <br /> 11. Provide information and referrals; <br /> 12. Implement a client grievance process; <br /> 13. Provide a Portable Toilet and Service at least once per week <br /> 14. Provide funding to support garbage disposal if use surpasses regular church pick up <br /> schedule <br /> 15. Install fencing around the designated parking area and provide new asphalt striping <br /> to indicate client parking spaces. <br /> 16. Be responsible for removing clients who don't comply with the rules from <br /> the Program and parking lot. CVPC reserves the right to ask for the removal <br /> of any client at its sole discretion. Ifclient removal is requested, CVPC will <br /> notify the Interfaith Family Shelter of such action. <br /> 17. Provide required data and reports to the City of Everett; <br /> 2 <br />
