18. Update 211 service with Program information; <br /> 19. Identify a contact person(s) who will be available to ensure that <br /> communication between the two organizations will be accurate and timely; <br /> 20. Meet with CVPC representatives at least monthly; <br /> 21. Communicate with CVPC within one business day when the following events <br /> occur: <br /> a. New client assigned a parking space, <br /> b. An incident, <br /> c. A grievance, <br /> d. Client leaves the Program, <br /> e. 911 call related to Program. <br /> 22. Maintain a binder at CVPC containing current copies of all forms and <br /> Program information; <br /> 23. Provide staff consultations both individually and at staff meetings; <br /> 24. Provide a "Certificate of Liability Insurance" naming CVPC as an additional <br /> insured and sign a hold harmless agreement; Minimum insurance <br /> requirements are as follows: <br /> Indemnify and Hold Harmless CVPC as follows: <br /> a. Except for the exclusion of two specific types of claims described below, <br /> Interfaith Family Shelter agrees to indemnify, defend, and hold <br /> harmless CVPC and its directors, officers, employees, members, and <br /> agents from any and all claims(which shall include without limitation, <br /> demands actions lawsuits and causes of action) <br /> arising from or by reason of or in any way growing out of the operation <br /> of the Program on CVPC property, Vvhether asserted by a participant, the <br /> City of Everett, or any third party (including without limitations any <br /> neighboring or nearby property owner or resident), and all liability, <br /> damages, legal fees, and costs(which included investigation, <br /> depositions, administrative, and expert witness fees and costs)related or <br /> arising from such claims. <br /> b. Interfaith Family Shelter, Agreement to Indemnify and Hold Harmless <br /> CVPC shall not cover or apply to two specific types of claims that would <br /> be covered by existing insurance of CVPC.
