The residential management office of <br /> Everett is accessible to persons with disabilities. Accessibility for the hearing impaired is <br /> provided by TDD/TDY telephone numbers. <br /> CHS's policies and practices will be designed to provide assurances that all persons with <br /> disabilities will be provided reasonable accommodation so that they may fully access and utilize <br /> the housing program and related services. CHS will work with the resident and their CCS <br /> housing case manager to make reasonable accommodation. The availability of specific <br /> accommodations will be made known by including notices on CHS forms/letters to all <br /> individuals, and all requests will be verified so that CHS can properly accommodate the need <br /> presented by the disability. <br /> Requests for reasonable accommodation from persons with disabilities will be granted upon <br /> verification that they meet the need presented by the disability. Reasonable accommodation will <br /> be made for persons with a disability who require an advocate or accessible offices. A designee <br /> will be allowed to provide some information,but only with the permission of the person with the <br /> disability. <br /> Individuals claiming a disability or handicap who are not in receipt of Social Security Disability <br /> or Supplemental Security Income must submit a certification of the disability or handicap. The <br /> certification must be signed by a licensed physician or other appropriate professional. All <br /> required criteria must be met. <br /> • SECURITY AND EMERGENCY PLAN <br /> Security is accomplished in the building by: 24/7 front desk staffing, card reader entry system, <br /> well monitored guest policy procedures and a security camera system. Relationships will remain <br /> active with local Police, Fire and other emergency responders. A Safety Committee will meet <br /> monthly to assess any current needs and ensure that best practices are utilized. Weekly meetings <br /> of key site staff and bi-weekly meetings with all services providers ensure continuity of <br /> communication for effective coordination of services with a consistent goal of safety for <br /> residents,staff and general community. <br /> Emergency plans are developed using templates and procedures developed by CCS& CHS from <br /> years of operating similar facilities and programs. These plans are updated annually to keep up <br /> with recent developments in emergency responses and to refresh all staff's knowledge of proper <br /> procedures. An emergency contact and procedures binder is available on site and there is off-site <br /> staff with access to this information as well. <br /> 10 <br />
