Weekly Statement with Respect to Payment of Wages (Form WH347): Contractors, Subcontractors, and agents to Subcontractors using Payroll Form WH347) may use State of Compliance found on back of form. Contractors, Subcontractors, or agents to Subcontractors not using Payroll Form WH347 shall attach the Statement of Compliance Form WH348 to each payroll. Submittal of Certified Payrolls and Statements of Compliance is required for projects utilizing federal funds, or when requested in writing by the Engineer.
