I CITY OF EVERETT SPECIAL PROVISIONS Adjusting new catch basin inlets to final grade is incidental and shall be included in the unit ' price for this bid item. The cost of connection of pipes to the inlet or catch basin is incidental and shall be included in the unit price for this bid item. Measurement and payment shall be per each catch basin inlet installed as measured upon completion. Bid Item 37 &96: Cement Concrete Pavement Base Type III (H.E.S.) for Trench Patch Measurement and Payment: Unit Price per Square Yard (SY) Measurement for Cement Concrete Pavement, (H.E.S.) shall be per square yard as delineated by the Contractor and approved by the Inspector prior to removal. The unit price per square yard for Cement Concrete Pavement Base (H.E.S), shall be full compensation for all materials, tools, labor and equipment necessary to complete the Work in accordance with the Plans, COE Standard Drawings, Section 5 of the Standard Specifications and 5-06.3(2)and 5-06.3(3) of these Special Provisions. Removal and disposal of existing concrete base and subbase within the area to be patched is also included in this item. Placement of through joints and dummy joints to match existing or as directed by the Engineer,and placement of approved curing compound is to be included in this bid item with no direct compensation made. Crushed surfacing base course will be measured and paid separately. Bid Item 38 & 97: HMA, for Permanent Trench Patch Measurement and Payment: Unit Price per Ton (TON) Measurement for HMA,for Permanent Trench Patch shall be by the ton, recorded on certified weight tickets in accordance with 1-09.2 WEIGHING EQUIPMENT for HMA placed within the limits of dimensions defined in the Work or shown on the Plans, Standard Specifications, Section 5-06 of these Special Provisions, or as otherwise approved by the Engineer. The measurement and payment limits for measuring the width of this bid item shall not exceed the saw-cut limits shown on the Drawings. No deduction will be made for the weight of asphalt binder, blending sand, mineral filler, or other component of the mixture. 111 The unit price for this bid item shall include all labor, materials, equipment and related work necessary to furnish, machine-place, compact, roll and perform density tests on multiple lifts of HMA—CL 1/2" PG 64-22 pavement including streets, alleys,street crossings, and driveway aprons as shown on the Plans. Miscellaneous HMA used for pavement repair, driveways, thickened edges, extruded and wedge curbing, placing and compacting pre-leveling, leveling and wearing courses, feathering pavement is included in this bid item with no direct compensation made. Furnishing and applying tack coat, prime coat, joint seal and crack sealing asphalt is to be included in this bid item with no direct compensation made. All costs for "Anti Stripping Additive" and "Compaction Adjustment" shall be included in the unit contract price per ton of the HMA with no direct compensation made. The unit price for this bid item shall include all labor, materials, equipment and related work necessary to furnish, place, compact, roll and perform density tests on ATB placed in accordance with the Plans, Standard Specifications, and these Special Provisions. Bid Item 39 &98: HMA, for Overlay 111 Measurement and Payment: Unit Price per Ton (TON) Measurement for HMA, for Overlay shall be by the ton, recorded on certified weight tickets in accordance with 1-09.2 WEIGHING EQUIPMENT for HMA placed within the limits of dimensions defined in the Work or shown on the Plans, Standard Specifications, Section 5- 06 of these Special Provisions, or as otherwise approved by the Engineer.
