I CITY OF EVERETT SPECIAL PROVISIONS Iand payment limits for measuring the width of this bid item shall not exceed the saw-cut limits shown on the Drawings. No deduction will be made for the weight of asphalt binder, blending sand, mineral filler, or other component of the mixture. IThe unit price for this bid item shall include all labor, materials, equipment and related work necessary to furnish, machine-place, compact, roll and perform density tests on multiple lifts of HMA— CL 1/2" PG 64-22 pavement including streets, alleys, and street crossings, as Ishown on the Plans. Furnishing and applying tack coat, prime coat, joint seal and crack sealing asphalt is to be included in this bid item with no direct compensation made. All costs for "Anti Stripping I Additive" and "Compaction Adjustment"shall be included in the unit contract price per ton of the HMA with no direct compensation made. The unit price for this bid item shall include all labor, materials, equipment and related work I necessary to furnish, place, compact, roll and perform density tests on ATB placed in accordance with the Plans, Standard Specifications, and these Special Provisions. Bid Item 40 &99: Planing Bituminous Pavement (2" Deep) IMeasurement and Payment: Unit Price per Square Yard (SY) Measurement for Planing Bituminous Pavement (2" Deep) shall be per square yard as delineated by the Contractor and approved by the Inspector. The unit price for this bid item I shall be full payment for all costs incurred to perform the Work described in Section 5- 04.3(14), to complete the work in accordance with the Plans, COE Standard Drawings, Standard Specifications and these Special Provisions. I Bid Item 41 & 100: Cement Concrete Sidewalk Restoration Measurement and Payment: Unit Price per Square Yard (SY) Measurement shall be per square yard as delineated by the Contractor and approved by the 1 Inspector prior to removal. The unit price per square yard for restoration of existing concrete sidewalks shall be full compensation for final sawcut and all materials, tools, labor and equipment necessary to I complete the work in accordance with the Plans, COE Standard Drawings, Standard Specifications and these Special Provisions. Bid Item 42: Cement Concrete Driveway Restoration IMeasurement and Payment: Unit Price per Square Yard (SY) Measurement shall be per square yard as delineated by the Contractor and approved by the I Inspector prior to removal. The unit price per square yard for restoration of existing concrete driveways shall be full compensation for all materials, tools, labor and equipment necessary to complete the Work I in accordance with the Plans, COE Standard Drawings, Standard Specifications, and these Special Provisions. Removal and disposal of existing driveway and base materials is also included in this item. Crushed surfacing top course placed under driveway is incidental to the driveway restoration item. IBid Item 43: Cement Concrete Curb and Gutter, Type A-1 Measurement and Payment: Unit Price per Linear Foot(LF) 1 Measurement shall be per linear foot as delineated by the Contractor and approved by the Inspector prior to removal. The unit price per linear foot for restoration of concrete curbs or curb and gutter and shall be I full compensation for all materials, tools, labor and equipment necessary to complete the Work in accordance with the Plans, COE Standard Drawings, Standard Specifications and these Special Provisions, including sawcutting, removal, loading, hauling and disposal.
