1 A certification of materials origin will be required for any items comprised of, or containing, I <br /> 2 steel or iron construction materials prior to such items being incorporated into the <br /> 3 permanent work. The certification shall be on DOT Form 350-109EF provided by the I <br /> 4 Engineer, or such other form the Contractor chooses, provided it contains the same <br /> 5 information as DOT Form 350-109EF. <br /> 6 <br /> 7 1-06.6.RTF <br /> 8 1-06.6 Recycled Materials <br /> 9 (January 4, 2016 APWA GSP) <br /> 10 <br /> 11 Delete this section, including its subsections, and replace it with the following: <br /> 12 <br /> 13 The Contractor shall make their best effort to utilize recycled materials in the construction I <br /> 14 of the project. Approval of such material use shall be as detailed elsewhere in the <br /> 15 Standard Specifications. <br /> 16 i 17 Prior to Physical Completion the Contractor shall report the quantity of recycled materials <br /> 18 that were utilized in the construction of the project for each of the items listed in Section <br /> 19 9-03.21. The report shall include hot mix asphalt, recycled concrete aggregate, recycled <br /> 20 glass, steel furnace slag and other recycled materials (e.g. utilization of on-site material <br /> 21 and aggregates from concrete returned to the supplier). The Contractor's report shall be <br /> 22 provided on DOT form 350-075 Recycled Materials Reporting. I <br /> 23 <br /> 24 1-07.GR1 <br /> 25 Legal Relations and Responsibilities to the Public <br /> 26 <br /> 27 1-07.1.RTF <br /> 28 1-07.1 Laws to be Observed <br /> 29 (October 1, 2005 APWA GSP) <br /> 30 <br /> 31 Supplement this section with the following: <br /> 32 <br /> 33 In cases of conflict between different safety regulations, the more stringent regulation <br /> 34 shall apply. <br /> 35 <br /> 36 The Washington State Department of Labor and Industries shall be the sole and <br /> 37 paramount administrative agency responsible for the administration of the provisions of <br /> 38 the Washington Industrial Safety and Health Act of 1973 (WISHA). <br /> 39 <br /> 40 The Contractor shall maintain at the project site office, or other well known place at the <br /> 41 project site, all articles necessary for providing first aid to the injured. The Contractor <br /> 42 shall establish, publish, and make known to all employees, procedures for ensuring <br /> 43 immediate removal to a hospital, or doctor's care, persons, including employees, who <br /> 44 may have been injured on the project site. Employees should not be permitted to work <br /> 45 on the project site before the Contractor has established and made known procedures <br /> 46 for removal of injured persons to a hospital or a doctor's care. <br /> 47 <br /> 48 The Contractor shall have sole responsibility for the safety, efficiency, and adequacy of <br /> 49 the Contractor's plant, appliances, and methods, and for any damage or injury resulting <br /> 50 from their failure, or improper maintenance, use, or operation. The Contractor shall be <br /> 51 solely and completely responsible for the conditions of the project site, including safety <br /> 52 for all persons and property in the performance of the work.
