I1 Removal and disposal of biological and physical hazards; removal of individuals <br /> 2 and encampments; removal, storage, and return of personal property; disposal <br /> I 3 of unclaimed personal property; additional site assessment, notifications, worker <br /> 4 training and personal protective equipment required after the initial site cleanup <br /> 5 is completed; and site preservation Work authorized by the Engineer will be paid <br /> I6 7 for by force account in accordance with Section 1-09.6. <br /> 8 For the purpose of providing a common proposal for all bidders, the Contracting <br /> 9 Agency has entered an amount for the item "FA-Site Cleanup of Bio. And <br /> I 10 Physical Hazards" in the bid proposal to become a part of the total bid by the <br /> 11 Contractor. <br /> 12 <br /> I 13 1-07.4(2),INST2.GR1 <br /> 14 Section 1-07.4(2) is supplemented with the following: <br /> 15 <br /> I 16 1-07.4(2).OPT2.GR1 <br /> 17 (April 7, 2020) <br /> 18 COVID-19 Health and Safety Plan (CHSP) <br /> I 19 20 The Contractor shall prepare a project specific COVID-19 health and safety plan <br /> (CHSP). The CHSP shall be prepared and submitted as a Type 2 Working Drawing <br /> 21 prior to beginning physical Work. <br /> I 22 <br /> 23 The Contractor shall update and resubmit the CHSP as the work progresses and <br /> 24 new activities appear on the look ahead schedule required under Section 1-08.3(2)D. <br /> 25 If the conditions change on the project, or a particular activity, the Contractor shall <br /> I26 update and resubmit the CHSP. Work on any activity shall cease if conditions prevent <br /> 27 full compliance with the CHSP. <br /> 28 <br /> I29 The CHSP shall address the health and safety of all people associated with the <br /> 30 project including State workers in the field, Contractor personnel, consultants, project <br /> 31 staff, subcontractors, suppliers and anyone on the project site, staging areas, or <br /> I32 33 yards. The plan shall contain the following minimum elements: <br /> 34 1. The CHSP shall identify all standards, guidance, publications, and sources <br /> I 35 on which it is based. Those standards may include references to OHSA, <br /> 36 WISHA, and CDC publications that are current at the time the CHSP is <br /> 37 prepared. <br /> 38 <br /> I39 2. The CHSP shall identify a responsible individual from the Contractor who is <br /> 40 responsible for implementation of the CHSP. The individual(s) contact <br /> 41 information shall be listed in the CHSP. <br /> I42 <br /> 43 3. The CHSP shall specifically identify the project for which it is applicable, <br /> 44 and if applicable, shall address project work areas outside the project limits <br /> I45 46 such as staging areas or yards. <br /> 47 4. The CHSP shall identify the PPE and administrative and engineered <br /> I 48 controls necessary to maintain a safe site.
