Throughout the life of the Contract, if Board members become aware <br /> 5 of potential conflicts of interest, they shall be disclosed to the parties <br /> I 6 immediately. <br /> 7 <br /> 8 Establishment of the Board <br /> I 9 10 The Contracting Agency and Contractor shall meet prior to the start of Contract <br /> time to jointly select three Board nominees. If the pay item, "Disputes Review <br /> 11 Board" is added by change order, the Contracting Agency and Contractor shall <br /> 12 meet to select Board nominees after the change order is processed. <br /> I13 <br /> 14 The Contracting Agency and the Contractor shall provide to the Board nominees <br /> 15 a list of the firms directly and indirectly involved with the Project, including, but <br /> 1 16 not limited to designers, architects, engineers, professional service firms, <br /> 17 consultants, JV partners, subcontractors and suppliers, along with a listing of <br /> 18 key personnel of each. <br /> I 19 <br /> 20 Board nominees shall provide to the Contractor and Contracting Agency the <br /> 21 following information within 21 calendar days of nomination. Board nominees <br /> I 22 that are included on the Washington State Department of Transportation <br /> 23 "Statewide Prequalified DRB Candidate Roster" will not be required to submit <br /> 24 resumes. <br /> I 25 <br /> 26 1. Resume showing: <br /> 27 <br /> 28 a. Full name and contact information <br /> I29 <br /> 30 b. Experience qualifying the person as a Board member as outlined <br /> 31 in the Board Member Qualifications subsection of this Special <br /> I 32 Provision. <br /> 33 <br /> 34 c. Previous Board participation, if any. List each Board assignment <br /> I 35 separately, indicating the name and location of the project, <br /> 36 approximate dates of Board service, name of Contracting Agency, <br /> 37 name of Contractor, names of the other Board members and the <br /> I <br /> 38 39 approximate number of disputes heard. When previous Board <br /> experience is extensive, the list may be truncated at the <br /> 40 prospective Board member's discretion. <br /> I 41 <br /> 42 2. Disclosure statement addressing the following: <br /> 43 <br /> 44 a. Previous or current direct employment by one of the parties <br /> I45 directly or indirectly involved. <br /> 46 <br /> 47 b.
