Throughout the life of the Contract, if Board members become aware 5 of potential conflicts of interest, they shall be disclosed to the parties I 6 immediately. 7 8 Establishment of the Board I 9 10 The Contracting Agency and Contractor shall meet prior to the start of Contract time to jointly select three Board nominees. If the pay item, "Disputes Review 11 Board" is added by change order, the Contracting Agency and Contractor shall 12 meet to select Board nominees after the change order is processed. I13 14 The Contracting Agency and the Contractor shall provide to the Board nominees 15 a list of the firms directly and indirectly involved with the Project, including, but 1 16 not limited to designers, architects, engineers, professional service firms, 17 consultants, JV partners, subcontractors and suppliers, along with a listing of 18 key personnel of each. I 19 20 Board nominees shall provide to the Contractor and Contracting Agency the 21 following information within 21 calendar days of nomination. Board nominees I 22 that are included on the Washington State Department of Transportation 23 "Statewide Prequalified DRB Candidate Roster" will not be required to submit 24 resumes. I 25 26 1. Resume showing: 27 28 a. Full name and contact information I29 30 b. Experience qualifying the person as a Board member as outlined 31 in the Board Member Qualifications subsection of this Special I 32 Provision. 33 34 c. Previous Board participation, if any. List each Board assignment I 35 separately, indicating the name and location of the project, 36 approximate dates of Board service, name of Contracting Agency, 37 name of Contractor, names of the other Board members and the I 38 39 approximate number of disputes heard. When previous Board experience is extensive, the list may be truncated at the 40 prospective Board member's discretion. I 41 42 2. Disclosure statement addressing the following: 43 44 a. Previous or current direct employment by one of the parties I45 directly or indirectly involved. 46 47 b.
