CITY OF EVERETT SPECIAL PROVISIONS <br /> At or prior to the preconstruction meeting, the contractor shall provide one <br /> of the following mix design verification certifications for Contracting Agency <br /> review; <br /> 1. The proposed mix design indicated on a WSDOT mix design/anti- <br /> strip report that is within one year of the approval date <br /> 2. The proposed HMA mix design submittal (Form 350-042) with the <br /> seal and certification (stamp & signature) of a valid licensed <br /> Washington State Professional Engineer. <br /> 3. The proposed mix design by a qualified City or County laboratory <br /> mix design report that is within one year of the approval date. • <br /> The mix design will be performed by a lab accredited by a national <br /> authority such as Laboratory Accreditation Bureau, L-A-B for Construction <br /> Materials Testing, The Construction Materials Engineering Council <br /> (CMEC's) ISO 17025 or AASHTO Accreditation Program (AAP) and shall <br /> supply evidence of participation in the AASHTO Material Reference <br /> Laboratory (AMRL) program. <br /> At the discretion of the Engineer, agencies may accept mix designs verified <br /> beyond the one-year verification period with a certification from the <br /> Contractor that the materials and sources are the same as those shown <br /> on the original mix design. <br /> Evaluation of anti-strip additives are to be provided as part of the mix <br /> design acceptance criteria. Acceptable anti-strip evaluations include 1.) a <br /> WSDOT validated mix design showing the validated anti-strip additive and <br /> dosage 2.) an historic anti-strip determination from WSDOT not greater <br /> than two (2) calendar years old or 3.) a passing TSR test at the anti-strip <br /> dosage proposed by the Contractor. <br /> No paving shall begin prior to Contracting Agency approval of the <br /> Contractor provided mix design. <br /> 5-04.3(7)A3 Field Verification Testing Process <br /> The City will collect three Production Samples of HMA on the first day of paving <br /> per AASHTO T 168 sampling procedures. <br /> a. The City will test one Production Sample in accordance with 5- <br /> 04.3(8)A for field verification per the requirements of 9-03.8(7). <br /> b. If the test results from the first Production Sample are within the <br /> tolerances of 9-03.8(7), the mix design will be considered verified and 40. <br /> the test results will be used as acceptance sample number one. <br /> c. If the test results from the first Production Sample are outside the <br /> tolerances of 9-03.8(7), the other two samples will be tested and the <br /> results of all three tests will be used for acceptance in accordance <br /> with 5-04.5(1) and will be used in the calculation of the CPF the <br /> maximum CPF shall be 1.00. <br /> Prior to the first day of paving, six Ignition Furnace Calibration Samples shall <br /> be obtained to calibrate the Ignition Furnaces used for acceptance testing of <br /> the HMA. Calibration samples shall be provided by the Contractor when <br /> directed by the Engineer.
