D. Levels of Accomplishment- Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subrecipient <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Rehabilitate 4 public <br /> bathrooms at Dawson Number of Clients Served 1, 100 <br /> Place Child Advocacy <br /> Center. <br /> Total unduplicated Everett Residents or <br /> Total People or Households to be served Households to be served <br /> 1, 100 Approximately 275 <br /> `Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subrecipient agrees to track outcome performance <br /> measures as required by the U.S. Department of Housing <br /> and Urban Development (HUD) and the City. The <br /> Subrecipient will report the outcomes to the City quarterly at <br /> minimum. <br /> Reporting forms must be in the format acceptable to the City of Everett. Exhibit "E" shall be used <br /> for all general reporting requirements under this agreement. <br /> E. Staffing <br /> [List of staff and time commitments to be allocated to the activity specified in l.A. above] <br /> General Administrative Performed in Grant Application Process (already incurred) <br /> •Grant Writer-40 hours includes research, accumulation of preliminary bids, budget development, <br /> writing narrative, and project meetings (internal and with the City). <br /> • Building Manager- 14 hours to organize and perform project walk-throughs with potential <br /> bidders. <br /> • Executive Director- 1 0 hours in grant application management and meeting with the City. <br /> Projected General Project Administrative Support Activities <br /> •Executive Director- 0.1 FTE (Average 4 hours/week) to head the Facility Bathroom Improvement <br /> Project from grant award to project completion <br />
