Project: Sewer"0"Utility Upgrades <br /> Change Order No. 1 <br /> Change Order Effective Date: May 4,2021 <br /> Bid Item#37 `—Cement Concrete Pavement Base Type III(High Early Strength)For Trench <br /> Patch. Additional square yards of Cement Concrete Pavement Base Type III (High Early Strength)For <br /> Trench Patch were required to restore Colby Avenue street surfacing as much larger areas of Colby <br /> Avenue street were needing to be removed and replaced because of the sides of the pipe trenches <br /> caving in during pipe installation, as a result of the soft,moist soil conditions encountered during pipe <br /> installation. <br /> Bid Items#40c—Planing Bituminous Pavement(2-inch deep). The City of Everett has elected <br /> to delete the work involved in Planing Bituminous Pavement(2-inch deep), as part of this contract and <br /> add that work to the annual 2021 Pavement Maintenance Overlay project. <br /> Bid Items#41 d—Cement Concrete Sidewalk Restoration. The number of square yards of <br /> Cement Concrete Sidewalk Restoration underran the original bid proposal amount,resulting in a <br /> contract price decrease. <br /> Bid Item#48—Adjust Existing Monument Casing to Grade. This bid item is being deleted as the <br /> existing survey monuments being excavated near required complete removal and reinstallation,as a <br /> result of the soft,moist soil conditions encountered during pipe installation. <br /> Bid Item#51 —Plastic Stop Line. <br /> Bid Item#52—Plastic Crosswalk Line. <br /> Bid Item#53—Removing Plastic Crosswalk Line. <br /> Bid Item#54—Removing Plastic Stop Line. <br /> Bid Item#55—Plastic Arrow. <br /> Bid Item#56—Removing Plastic Arrow. <br /> Bid Item#57—Plastic Line. <br /> Bid Item#58—Plastic Wide Line. <br /> Work under Bid Items#51 thru#58 are being deleted as the three blocks of Colby Avenue from 37th to <br /> 401h Streets are being added to the annual 2021 Pavement Maintenance Overlay project. <br /> Bid Item#60 g—Force Account. The amount of work anticipated to be tracked and paid for <br /> under the force account method,was less in dollar value than what the original bid proposal dollar <br /> values anticipated, resulting in a decrease to the contract dollar value. <br /> Bid Item#70—Saw Cutting Existing Pavement. The number of linear feet of Saw Cutting Existing <br /> Pavement was less in quantity than what the original bid proposal quantity anticipated,resulting in a <br /> decrease to the contract value. <br /> Bid Item#72—Crushed Surfacing Top Course. The number of tons of Crushed Surfacing Top Course <br /> was less in quantity than what the original bid proposal quantity anticipated,resulting in a decrease to <br /> the contract value. <br /> Bid Item#98—HMA,for Overlay.
