• Project: Sewer"0"Utility Upgrades Change Order No. 1 Change Order Effective Date: May 4,2021 Bid Item#37 &#96—Cement Concrete Pavement Base Type III(High Early Strength)For Trench Patch. Additional square yards of Cement Concrete Pavement Base Type III (High Early Strength)For Trench Patch were required to restore Colby Avenue street surfacing as much larger areas of Colby Avenue street were needing to be removed and replaced because of the sides of the pipe trenches caving in during pipe installation, as a result of the soft,moist soil conditions encountered during pipe installation. Bid Items#40 &#99—Planing Bituminous Pavement(2-inch deep). The City of Everett has elected to delete the work involved in Planing Bituminous Pavement(2-inch deep),as part of this contract and add that work to the annual 2021 Pavement Maintenance Overlay project, Bid Items#41 &#100—Cement Concrete Sidewalk Restoration. The number of square yards of Cement Concrete Sidewalk Restoration;underran the original bid proposal amount,resulting in a contract price decrease. Bid Item#48--Adjust Existing Monument Casing to Grade. This bid item is being deleted as the existing survey monuments being excavated near required complete removal and reinstallation,as a result of the soft,moist soil conditions encountered during pipe installation, Bid Item#51 —Plastic Stop Line, Bid Item#52—Plastic Crosswalk Line. Bid Item#53—Removing Plastic Crosswalk Line. Bid Item#54—Removing Plastic Stop Line. Bid Item#55—Plastic Arrow. Bid Item#56—Removing Plastic Arrow. Bid Item#57—Plastic Line. • Bid.Item#58—Plastic Wide Line. Work under Bid Items#51 th ru#58 are being deleted as the three blocks of Colby Avenue from 37th to 401h Streets are being added to the annual 2021 Pavement Maintenance Overlay project. Bid Item#60&#103—Force Account. The amount of work anticipated to be tracked and paid for under the force account method,was less in dollar value than what the original bid proposal dollar values anticipated,resulting in a decrease to the contract dollar value. Bid Item#70—Saw Cutting Existing Pavement. The number of linear feet of Saw Cutting Existing Pavement was less in quantity than what the original bid proposal quantity anticipated,resulting in a decrease to the contract value, Bid Item#72—Crushed Surfacing Top Course, The number of tons of Crushed Surfacing Top Course was less in quantity than what the original bid proposal quantity anticipated,resulting in a decrease to the contract value, Bid Item#98—HIM,for Overlay. The work to grind&overlay Colby Avenue between 37th&40th streets is being deleted from this contract and will be completed on the upcoming 2021 Pavement Maintenance Overlay project. Page 7 of 8
