Project: Sewer"0"Utility Upgrades <br /> Change Order No. 1 <br /> Change Order Effective Date: May 4,2021 <br /> New Item#106—Survey Monument Case&Cover. This new item was added to cover the costs to <br /> install new survey monument case&covers once all underground work and roadway surfacing was <br /> completely restored, <br /> New Item#107—Deletion of Traffic Loop Installations a 3711'&39th of Colby Avenue. This item is <br /> being deleted from this project and the work to install new traffic loops at the intersections of 37'1'& <br /> 39'h&Colby Avenue will be added to the 2021 Pavement Maintenance Overlay project. <br /> New Item#108—Equitable Adjustment. A changed condition due to soft,moist soils on Colby <br /> Avenue resulted in the sides of trenches collapsing into the trench,during sewer main and side sewer <br /> pipe trench excavation. This resulted in additional structural roadway excavation removals,soil <br /> excavation removals,haul costs,and significantly slowed completion of the project resulting in <br /> additional working days to be added to the Contract Time. Once the Change of Condition was known, <br /> the Contractor provided the Owner written Notice, as is a Contract requirement,when seeking added <br /> costs. Toward the end of construction,oncerthe Contractor had established the cost impacts resulting <br /> from this Changed Condition situation,submitted an Equitable Adjustment request in the amount of <br /> $348,562.18. City Construction Management upon receipt of this Equitable Adjustment request, • <br /> • <br /> review it and negotiated an Equitable Adjustment settlement,in the Lump Sum amount of <br /> $23 0,607.48. <br /> CONTRACT TIME: Thirty(30)additional working days are being added to the Contract Time as a <br /> result of the Changed Conditions of encountering of soft,moist soils while installing sanitary and <br /> storm sewer pipe in the first two blocks in Colby between 37'h and 39th streets,and stumps,logs and <br /> peat soils in the intersections of 39U'&40'h streets of Colby Avenue. <br /> Page 8 of 8 <br />
