A list of assumptions and clarifications made by the Design-Builder in the <br /> preparation of the Phase 2 Proposal. <br /> d. Identification of permits and approvals to be obtained by the Design-Builder <br /> during Phase 2, if any. <br /> 2. Project Management submittal, including: <br /> a. Proposed Phase 2 Project Schedule including an accompanying narrative <br /> describing key assumptions in the proposed Project Schedule upon which the <br /> Guaranteed Maximum Price (GMP) is based, and including dates for <br /> Substantial Completion and Final Completion. <br /> b. Proposed Phase 2 Health and Safety Plan <br /> c. Proposed Phase 2 Quality Management Plan <br /> d. Revised Self-Performance and Subcontracting Plan, identifying work <br /> packages planned to be self-performed and subcontracted, whether those <br /> packages are being procured competitively or sole sourced, description of <br /> approach to procurement for competitive packages, and identity of <br /> subcontractor for sole sourced packages. <br /> e. Descriptive summary of all engineering, procurement, materials, construction <br /> labor and equipment, and other services necessary to perform the Phase 2 <br /> Work as required under this PDB Contract. <br /> f. A Phase 2 scope of work for engineering services during construction, <br /> describing the work tasks and activities to be provided by the Engineer of <br /> Record and other engineering or geotechnical firms during Phase 2. <br /> g. A Phase 2 staffing plan describing the construction oversight activities to be <br /> completed by the Design-Builder, by the Engineer of Record, by other <br /> engineering or geotechnical firms, and by the City. The intent of this <br /> document is to demonstrate a shared understanding of oversight roles and <br /> responsibilities during construction, and to minimize duplication of efforts. <br /> 3. Pricing submittal, including: <br /> a. The proposed Cost of the Phase 2 Work as defined in the PDB Contract <br /> (including all services required for construction of the Project through Final <br /> Completion) using the City-approved cost model. <br /> b. Supporting documentation for the proposed Cost of the Phase 2 Work at a <br /> minimum shall include: <br /> i. Subcontractor and materials vendor bids and quotations <br /> ii. Details supporting estimates for self-performed construction work (labor, <br /> materials and equipment) <br /> iii. Expense rates such as mileage charges, per diem for meals and lodging, <br /> and personnel vehicle rentals; <br /> iv. Unburdened rental rates on construction equipment, trailers, storage and <br /> staging space and major tools; <br /> v. Allowances (where appropriate) <br /> Page 14 of 15 <br />
