weekly check-ins, review of circumstances and relationship building we will determine <br /> ongoing ability to maintain current housing past the immediate need and connect client <br /> with additional community resources which includes food banks, programs for low- <br /> moderate income utility programs and feeding programs. <br /> C. Performance Monitoring <br /> The City will monitor the performance of the Subrecipient against goals and performance standards <br /> required herein through quarterly reports and site visits. Substandard performance as determined by <br /> the City will constitute non-compliance with this Agreement. If action to correct such substandard <br /> performance is not taken by the Subrecipient within a reasonable period of time, as determined by <br /> the City and after being notified by the City, contract suspension or termination procedures will be <br /> initiated. <br /> D. Levels of Accomplishment-Goals and Performance Measures <br /> In addition to the normal administrative services required as part of this Agreement, the Subrecipient <br /> agrees to provide the following levels of program services: <br /> Activity Units of Service* per Month Total Units/Year <br /> Number of households <br /> who have been provided 5.0 60 <br /> with rental assistance <br /> Total People or Households to be served Total unduplicated Everett Residents or <br /> Households to be served <br /> 60 Households 60 Households <br /> *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, <br /> persons or households assisted, or meals served, here. If this is a construction project, please <br /> explain here how the improvements will be used and how they benefit the community, what the <br /> target population is for the project, etc. <br /> The Subrecipient agrees to track outcome performance measures as required by the U.S. <br /> Department of Housing and Urban Development(HUD) and the City. The Subrecipient will report <br /> the outcomes to the City on the following schedule: <br /> Performance Period Report Due <br /> May 1, 2021 — December 31, 2021 January 31, 2022 <br /> January 1, 2022— March 31, 2022 April 29, 2022 <br />
