Location: Preceding action: Fund: Self -Insured Fund Fiscal summary statement: The fees for the proposed contract term of March 1, 2023 — February 28, 2026 are variable and subject to adjustment per the fee breakdown below: • Annual Admin Fee: $2.40 per eligible employee per year • Case Rate Fee: $2,200.00 for each engaged member per year The total expected annual cost for this service based on utilization and the administrative fee is approximately $75,240.00. Depending on employee participation a budget amendment may be needed in late fall, of 2023. Project summary statement: Carrot Fertility, Inc. provides services to support employees and their families through their unique family -planning journeys. Carrot provides family planning services, infertility resources, and subject matter experts for support throughout pregnancy and post-partum. Services from Carrot will be available to all benefit eligible employees and eligible dependents.
