<br />City of Everett – Replacement of Reservoir 3 14 Scope of Services <br />Subtask 700.3 – Permitting Support Services <br />Consultant Services <br />1. Conduct a kickoff workshop with the Department of Health. <br />2. Prepare the Project Report for the selected reservoir alternative for submittal to DOH. <br />The Project Report will include the PDR and other relevant supporting information <br />developed under Task 200. The Report will meet the requirements of WAC 246-290-10 <br />and the DOH Water System Design Manual. <br />3. Address and respond to DOH review comments for the Project Report. <br />4. Submit Phase 1 90% Design to the DOH in accordance with WAC 246-290-120 and <br />provide responses to comments. <br />5. Submit Phase 1 Construction Documents to the DOH in accordance with WAC 246-290- <br />120. <br />6. Submit Phase 2 90% Design to the DOH in accordance with WAC 246-290-120 and <br />provide responses to comments. <br />7. Submit Phase 2 Construction Documents to the DOH in accordance with WAC 246-290- <br />120. <br />8. Submit DOH Construction Completion Report (CCR) for Phase 1 construction. <br />9. Submit DOH Construction Completion Report (CCR) for Phase 2 construction. <br />10. Assist the City in the acquisition of other permits required for the Project. <br />Client Responsibilities <br />1. Schedule and participate in the DOH kickoff meeting. <br />2. Responsible to prepare and obtain all permits required for construction. <br />3. City to request support from HDR in preparation of permits on an as-needed basis. <br />Assumptions: <br />1. Kickoff workshop with DOH to be held two weeks following NTP. This will be an in- <br />person kickoff held at the City offices. This workshop will be four hours in duration and <br />will be attended by the HDR Project Manager and Deputy Project Manager. <br />2. An allowance of $20,000 has been allocated to assist the City in obtaining permits for <br />the Project. <br />Deliverables <br />1. Kickoff Meeting Agenda and Notes. <br />2. Draft and Final DOH Project Report in accordance with WAC 246-290-110. <br />3. Phase 1 and 2 CCRs.
