I <br /> 1 • The DBE itself shall own and operate at least one fully licensed, <br /> 2 insured, and operational truck used on the Contract. The drivers of the <br /> 3 trucks owned and leased by the DBE must be exclusively employed by <br /> 4 the DBE and reflected on the DBE's payroll. <br /> I5 <br /> 6 • Lease agreements for trucks shall indicate that the DBE has exclusive <br /> 7 use of and control over the truck(s). This does not preclude the leased <br /> 8 truck from working for others provided it is with the consent of the DBE <br /> 9 and the lease provides the DBE absolute priority for use of the leased <br /> 10 truck. <br /> 1 11 <br /> 12 • Leased trucks shall display the name and identification number of the <br /> 13 DBE. <br /> I 14 <br /> 15 UDBE/DBE/FSBE Truck Unit Listing Log <br /> 16 In addition to the subcontracting requirements of Section 1-08.1, each DBE <br /> 17 trucking firm shall submit supplemental information consisting of a completed <br /> I18 Primary UDBE/DBE/FSBE Truck Unit Listing Log (WSDOT Form 350-077), copy <br /> 19 of vehicle registrations, and all Rental/Lease agreements (if applicable). The <br /> 20 supplemental information shall be submitted to the Engineer prior to any trucking <br /> I 21 services being performed for DBE credit. Incomplete or incorrect supplemental <br /> 22 information will be returned for correction. The corrected Primary <br /> 23 UDBE/DBE/FSBE Truck Unit Listing Log and any Updated Primary <br /> I 24 UDBE/DBE/FSBE Truck Unit Listing Logs shall be submitted and accepted by <br /> 25 the Engineer no later than ten calendar days of utilizing applicable trucks. <br /> 26 Failure to submit or update the DBE Truck Unit Listing Log may result in trucks <br /> I 27 not being credited as DBE participation. <br /> 28 <br /> 29 Each DBE trucking firm shall complete a Daily UDBE/DBE/FSBE Trucking Unit <br /> I 30 Listing Log for each day that the DBE performs trucking services for DBE credit. <br /> 31 The Daily UDBE/DBE/FSBE Trucking Unit Listing Log forms shall be submitted <br /> 32 to the Engineer by Friday of the week after the work was performed. <br /> 33 I 34 Joint Checking <br /> 35 A joint check is a check between a Subcontractor and the Contractor to the <br /> 36 supplier of materials/supplies. The check is issued by the Contractor as payer <br /> 37 to the Subcontractor and the material supplier jointly for items to be incorporated <br /> 38 into the project. The DBE must release the check to the supplier, while the <br /> 39 Contractor acts solely as the guarantor. <br /> I 40 <br /> 41 A joint check agreement must be approved by the Engineer and requested by <br /> 42 the DBE involved using the DBE Joint Check Request Form (form # 272-053) <br /> I 43 prior to its use. The form must accompany the DBE Joint Check Agreement <br /> 44 between the parties involved, including the conditions of the arrangement and <br /> 45 expected use of the joint checks. <br /> 46 I <br /> 47 The approval to use joint checks and the use will be closely monitored by the <br /> 48 Engineer.
