additional community resources which includes food banks, programs forlow-moderate <br />income utility programs and feeding programs. We require proof of income through pay <br />stubs or a letter from employer/s for every adult living in the home. We require proof of <br />state issued identification such as driver's license for all adults living in the home. We <br />verify that all clients are LMI according to HUD AMI guidelines. <br />C. Performance Monitorinq <br />The City will monitor the performance of the Subrecipient against goals and performance standards <br />required herein through quarterly reports and on-site and/or virtual site visits. Substandard <br />performance as determined by the City will constitute non-compliance with this Agreement. lf action <br />to correct such substandard performance is not taken by the Subrecipient within a reasonable period <br />of time, as determined by the City and after being notified by the City, contract suspension or <br />termination procedures will be initiated. <br />D. Levels of Accomplishment - Goals and Performance Measures <br />ln addition to the normal administrative services required as part of this Agreement, the Subrecipient <br />agrees to provide the following levels of program services: <br />Activity Units of Service* per Month Total Units/Year <br />Number of households <br />who have been provided <br />with rental- assistance <br />3.3 40 <br />Total People to be serued Total unduplicated Everett Residents to be <br />served <br />40 individuals 40 individuals <br />*þlþ: Provide definition of Units of Seruice, which may include such measures as unifs rehabbed, <br />persons or households assrsted, or meals serued, here. lf this is a construction project, please <br />explain here how the improvements will be used and how they benefit the community, what the <br />target population is for the project, etc. <br />The Subrecipient agrees to track outcome performance measures as required by the U.S. <br />Department of Housing and Urban Development (HUD) and the City. The Subrecipient will report <br />the outcomes to the City on the following schedule: <br />Performance Period Report Due <br />January 1, 2023 - June 30, 2023 July 31 ,2023
