SECTION 01 26 00 <br /> CONTRACT MODIFICATIONS <br /> PART 1 —GENERAL <br /> 1.1 SUMMARY <br /> A. This section includes the administrative and procedural requirements for executing a <br /> change in the Work as herein specified and further described in Part 7 of the General <br /> Conditions. <br /> B. Owner's forms referenced in this Section include (see Appendix A): <br /> 1. Proposal Request (PR); <br /> 2. Change Order; <br /> 3. PR General Contractor Breakdown Summary; <br /> 4. PR Subcontractor Breakdown Summary; <br /> 5. PR Cost Breakdown; <br /> 6. PR Wage Rates; <br /> 7. PR Equipment Rates; and, <br /> 8. Construction Change Directive <br /> 1.2 PRELIMINARY REQUIREMENTS: <br /> A. Prior to submitting Contractor's first Change Order Request (COR), or responding to the <br /> first Proposal Request (PR), the Contractor shall submit a breakdown of journeyman and <br /> apprentice wage rates using the PR Wage Rates form included in the Appendix. The <br /> breakdown shall show the following allowable wage rate cost components: <br /> 1. Basic wage rate (based on L&I Intent to Pay Prevailing Wages or union agreement); <br /> 2. Fringe Package (based on L&I Intent to Pay Prevailing Wages or union agreement); <br /> 3. FUI (Federal Unemployment Insurance); <br /> 4. FICA (Federal Insurance Compensation Act); <br /> 5. Medicare; <br /> 6. SUI (State Unemployment Compensation Act); <br /> 7.
