Injury and Illness Reporting <br />All injuries, no matter how minor, must be reported immediately. Supervisors are required to <br />prepare a company accident report of the injury, as well as any required DOSH reports. Injuries <br />should be investigated immediately. Written statements are to be taken from all witnesses and <br />supervisory personnel involved and photographs are to betaken as needed. It must be <br />emphasized that investigations are to be conducted with the goal of finding facts, not fault. <br />The Company Accident Report should be supplemented with notes of any conversations or <br />directions provided by competent medical authorities, including names, dates, times, instructions or <br />advice. <br />The following incidents must be reported to the Department of Labor and Industries at 1-800-4BE <br />SAFE within 8 hours of the work -related incident or accident: <br />A death <br />A probable death <br />2 or more employees are admitted to the hospital <br />f L&I is notavailable (i.e., after hours, weekends, or holidays) contactthe Occupational Safety <br />and Health Admi nistration (OSHA) by calling its central numberat 1-800-321-6742. You musttalk <br />to an actual person WITHIN 8 HOURS. <br />Do not move any equipment until a representative of the Department of Labor and Industries <br />investigates the incident and releases the equipment unless moving the equipment is necessary to: <br />Remove any victims. <br />Preventfurther incidents and injuries. <br />First Aid <br />Teknotherm will have available at each work site where a crew is present, a crew leader that has a <br />valid first aid certificate. First aid kits are available in each company vehicle, in eachjob box and at <br />the shop. Kits are clearly identifiable as first aid supplies, and checked monthly. f you use first aid <br />supplies, restock the kit using spare supplies kept at the shop, or let someone know sot can be <br />restocked. <br />Emergency eyewashes must be provided when there is potential for an employee's eyes to be <br />exposed to corrosives, strong irritants, or toxic chemicals. Emergency showers must be provided <br />when there is potential for major portions of an employee's body to contact corrosives, strong <br />irritants, or toxic chemicals. Use the MSDS to determine whether emergency flushing of skin or <br />eyes is required. When in the field, identify the locations of onsite eyewash stations and showers <br />before beginning work. <br />Employees involved in any emergency greater than a minorincident are expected to actas follows: <br />1. Before entering the area of an accident, protect yourself first. Survey the scene to be sure you <br />can enter safely. Evaluate what PPE (Personal Protective Equipment) you may need and <br />wear it. If you cannot determine if you can enter the area safely, then do not enter it. <br />
