Any other person who has experience and skills needed <br />A preliminary investigation includes noting information such as the following: <br />Where did the accident or incident occur? <br />Whattime did itoccur? What <br />people were present? <br />What was the employee doing at the time of the accident or incident? What <br />happened during the accident or incident? <br />Provide the following iformation to L&I concerning any accident involving a fataty or hospitalization <br />of 2 or more employees: <br />Name of the work place Location <br />of the incident <br />Time and date of the incident <br />Number of fatalities or hospitalized employees Contact <br />person <br />Phone number <br />Brief description of the incident <br />Safety Meetings <br />Teknotherm will conduct crew leader -crew safety meeting at each job site as follows: <br />1. The meetings will behdd daily at 6am <br />2. The meeting shall be tailored to the specificjob site <br />Generally meetings will cover: <br />1. A review of safety inspections <br />2. A review of accident investigations <br />3. A discussion about unsafe conditions <br />All employees are required to attend. The leader of the meeting will designate a person to <br />document attendance, meeting location and the topics discussed. Documentation of meetings will be <br />maintained at the office for one year. <br />All onsite subcontractors will be required to attend the site -specific safety meeting. <br />Workplace Inspection <br />At the beginning of each job, a walk -around safety inspection shall be conducted jointly by one <br />member of management and one employee. Walk around inspections will be conducted in the <br />shop/vessel at least monthly by a designated employee. In addition to walk around safety <br />inspections, supervisors will inspect work areas regularly to ensure safe conditions are maintained. <br />Teknotherm will document the walk -around safety inspections. The inspection documents will be <br />maintained until the completion of the job for field work. Shop/vessel inspections will be maintained <br />on filefor one year. <br />Supervisor Involvement <br />All supervisors shall participate in all aspects of safety.
