Category 2: Sensitive information <br />Page 11 of 23 Category 2: For official use only / disclosure permissible by law. <br />2.6 SAFETY DATA SHEETS <br />A copy of the most current SDS must be submitted to the city of Everett with the supplier’s proposal <br />submission, as well as with each delivery of Bioxide®. <br />2.7 REPORTS <br />Upon request, the supplier must provide the City service reports after each site visit. At a minimum, each <br />service report must include total chemical usage and recommended service to be completed at the site. <br />City staff will coordinate with the awarded supplier the information that will be included in each service <br />report. <br />2.8 SAFETY REQUIREMENTS <br />The material must not be listed as an Extremely Hazardous Substance under Section 302 of Emergency <br />Planning and Community Right-to-Know Act (EPCRA), nor be listed as a Comprehensive Environmental <br />Response, Compensation and Liability Act (CERCLA) hazardous substance. The material must be exempt <br />from Federal Department of Transportation (DOT) placard requirements. Recommended handling <br />procedures for the material must require protective gloves and safety glasses only. Any material <br />recommending more sophisticated equipment (i.e. face shield, body suit, etc.) during routine handling will <br />not be considered. <br />The Supplier will provide a copy of the current Safety Data Sheet (SDS) for the product offered at the time <br />of proposal. The SDS must include all chemical compounds present in concentrations greater than 0.1% <br />for each product offered by Chemical Abstract Service (CAS) number; no “trade secret” or otherwise <br />defined ingredients must be accepted by the City. The Supplier will provide required safety and health <br />training for City employees on the product offered and proper product use, storage, and disposal, when <br />requested by the City. <br />The Supplier will be responsible for the proper labeling of storage tanks for their product in compliance <br />with local, state, and federal requirements. <br />2.9 SUPPLIER REQUIREMENTS <br />The awarded supplier will be responsible for the following tasks <br />1. Coordinating delivery of Bioxide® with City of Everett Staff. <br />2. Performing quarterly services visits. <br />3. Performing maintenance on the chemical delivery systems. <br />4. Providing the City with quarterly service reports and monthly chemical monitoring reports. <br />5. Providing technical assistance that is available within 24-hours of notification. <br />6. Providing a copy of the current SDS for the product. <br />7. Following all local, state, and federal government safety and labeling requirements. <br />2.10 SINGLE SOURCE SUPPLIER <br />1. All aspects of this service contract must be provided by a single supplier who will have sole <br />responsibility for the product and service program. <br />2.11 CITY OF EVERETT RESPONSIBILITIES <br />The City of Everett staff is responsible for the following: <br />1. Determining when additional product is required and ordering product to meet needs within the <br />delivery timeline.
