B. The city clerk may by order limit the number of licenses issued under this ordinance when the city clerk determines it is necessary in order to protect the general health, safety and welfare. Section 9: For-hire vehicle business license-Application. In addition to the information required to be provided pursuant to the general requirements for all licenses, applicants for for-hire vehicle business licenses shall furnish the following information on forms to be furnished by the city, which information shall be signed and sworn to: A. The financial status of the applicant, including the amounts of all unpaid judgments and the nature of the transaction or acts giving rise to such judgments; B. The experience of the applicant in the transportation of passengers; C. The number of vehicles to be operated or controlled by the applicant; D. For each for-hire vehicle,the company vehicle number therefor,the make, model and identifying color scheme, monogram or insignia, and serial number of the vehicle; E. True and accurate information concerning the ownership of the vehicle(s)to be licensed; F. True and accurate information concerning the ownership of the for-hire vehicle business, including: 1. The form of business entity under which the business will operate; 2. If individually owned, the owner's full name, date of birth, residence address, residence telephone number, business street address,business post office box address if any, and business telephone number; 3. If a corporation, partnership or other business entity, the name, date of birth, residence address, residence telephone number, and business telephone number for all officers, directors, shareholders, general partners, limited partners, managing partners and all other persons vested with authority to manage or direct the affairs of the legal entity or to bind the legal entity in dealings with third parties or who have any ownership interest; 4. If a corporation, partnership or other business entity,the state of incorporation or registration with the Secretary of State of the State of Washington(if any) and State of Washington business license number; G. Proof of a valid state for-hire certificate(which may be submitted up to 30 days following granting of a city license); H. The name, fictitious or otherwise, under which the applicant will operate; I. The applicant's criminal history; and J. Such other information the city clerk may require, which he/she deems reasonably necessary to aid in the enforcement of this ordinance. Section 10: License fee. A. The license fees are fixed in the amounts shown in the following schedule and shall be payable annually in advance: 1. For-hire vehicle business license-One hundred fifty dollars per year per company plus seventy-five dollars per year for each for-hire vehicle; 5
