Action Item 3 — Some anchor reinforcing bars appeared to violate top mat clearances and should be re- <br /> positioned to maintain minimum cover requirements per the contract plans. See Figure 4. <br /> 4. Action Item 4 — Instances of insufficient side clear cover was noted at the rebar cage locations. After <br /> adjustment of the #11 bars (see items no. 1 thru 3), clear distances shall be re-checked to ensure meeting <br /> minimum clear cover requirements per the contract plans. See Figure S. <br /> 5. Action Item 5—Instances of loosened weather barrier sheeting on the sides of the foundation should be re- <br /> attached and fixed into place after rebar locations are finalized prior to the pour to prevent wet concrete <br /> from getting between the sheet and edge of foundation. See Figure 6. <br /> 6. Out-of-Conformance Item 1— Remnants of tie wire and mylar sheets that have fallen to the bottom of the <br /> excavated slab area should be cleared and removed prior to the concrete pour. <br /> 7. Out-of-Conformance Item 2—Instances of crossing bars in contact without mylar sheets between them were <br /> observed at a few locations on the top mat. Installation of mylar sheets at contact points between bars <br /> should be completed and reviewed by Helion prior to the concrete pour. See Figure 7. <br /> Additional Notes/In-Conformance Items: <br /> 1. General spacing of rebar, on average, was acceptable. <br /> 2. Footing dimensions, width, and depth, were acceptable. <br /> 3. Lap splice lengths were measured in field and were acceptable. <br /> 4. Top mat reinforcing and rebar cage detailing at the exposed building column locations except as noted <br /> in Action Items 1 thru 3 were acceptable. <br /> Hayden Leblonde of Andersen Construction and Paul Gentsch of Helion Energy were on site at the time of the <br /> observation and was notified of the above-mentioned action items.The additional notes were briefly discussed <br /> but have been expanded upon in this report with supplemental information.
